Analysis on the influencing factors of the clinical effects of conservative treatment for radial head fracture

2018
Objective To analyze the influencing factors of the clinical efficacy of conservative treatment for radial head fractures. Methods Sixty-three patients with radial head fracture were selected as research objects and were confirmed by X-ray examination and other auxiliary measures. All patients were treated by conservative treatment. The relationship of clinical effect with the general clinical factors such as gender, age, body mass index, and with the factors in treatment period such as the type of fracture, the time from injury to treatment. Results After 3 months of treatment, among the 63 patients, the imaging review score≥90 points in 27 cases, 80 to 89 points in 24 cases, 70 to 79 points in 5 cases, <60 points in 7 cases, the effective rate of imaging evaluation was 80.95%; the elbow joint function score≥21 points in 5 cases, 9 to 20 points in 8 cases, 3 to 8 points in 26 cases, 0 to 2 points in 24 cases, the excellent recovery rate of elbow joint was 79.37%; imaging examination score≥80 points, elbow joint function score≤8 points in 47 patients, conservative treatment efficiency was 74.60%. The age of patients over 65 years old, with a history of smoking, Mason type Ⅱ or type Ⅲ, the ineffective reset and long-term recovery training after treatment were independent risk factors for conservative treatment of patients with radial head fractures (P<0.05). Conclusions Over 65 year-old, having a history of smoking, Mason type Ⅱ or Ⅲ, poor response and long-term recovery training after treatment are independent risk factors for the clinical effect of conservative treatment in patients with radial head fractures. Conservative treatment is suitable for Mason type Ⅰ fracture patients. And it is necessary to pay special attention to the treatment for elderly patients to improve the reset effect and early implement recovery training.
