SUMMARY Purpose: To determine the efficiency and safety of the endovascular treatment in dissecting aneurysms of the posterior cerebral artery (PCA). Material and Methods: Between March 1995 and March 2004 7 consecutive patients with dissecting aneurysms of the P2 segment of the PCA were treated, 4 males and 3 females, ranging in age from 2 to 62 years. All the patients presented with subarachnoid hemorrhage. Three aneurysms were embolized and three parent artery proximal occlusion were performed with controlled detachable coils (GDC). In other case there was spontaneous thrombosis of the aneurysm. Results: Of the three patients whom were preformed parent artery proximal occlusion: two of them do not presented any neurological deficit and one developed hemianopsia. Treatment included endovascular placement of GDC and aneurysm occlusion on three other patients, two of whom without neurological deficit and one with an acute ischemic event in distal PCA territory that occurred 48 hours after the procedure was performed, with subsequent death of the patient. In one patient spontaneous thrombosis of the aneurysm occurred. Conclusion: Dissecting aneurysms of the PCA are rare and with poor outcome, with few reports on the literature.
