Association of Bone Mineral Density with Chronic Obstructive Pulmonary Disease in Postmenopausal Women

espanolAntecedentes: la osteoporosis (OP) es comun en pacientes con enfermedad pulmonar obstructiva cronica (EPOC). La relacion entre OP y EPOC se ha estudiado principalmente en pacientes masculinos, y hay pocos informes disponibles en mujeres posmenopausicas. Objetivo: El proposito de este estudio fue investigar la asociacion entre la densidad mineral osea (DMO) y la EPOC en mujeres posmenopausicas. Metodos: Este estudio transversal incluyo 133 mujeres ex fumadoras clinicamente estables con EPOC confirmada, y 31 controles femeninos "ex fumadores" de la misma edad. Analizamos grupos segun su categoria de obstruccion de las vias respiratorias. La DMO se midio en imagenes de absorciometria de rayos X de energia dual del cuello femoral izquierdo. Resultados: los pacientes con EPOC tenian puntuaciones T mas bajas y una mayor prevalencia de osteopenia / OP que el grupo control. En el grupo de EPOC, la categoria de obstruccion de las vias aereas se asocio significativamente con la puntuacion T despues del ajuste por factores de confusion. El analisis de regresion logistica multivariante mostro que la EPOC era un marcador independiente para un mayor riesgo de osteopenia / OP en mujeres posmenopausicas. Conclusiones: la categoria de EPOC y obstruccion de las vias aereas se relaciono fuertemente con la DMO. Las mujeres posmenopausicas con EPOC, especialmente aquellas con obstruccion grave de las vias respiratorias, tenian una tasa de prevalencia mas alta y un mayor riesgo de osteopenia y OP que los controles femeninos sin EPOC. EnglishBackground: Osteoporosis (OP) is common in patients with chronic obstructive pulmonary disease (COPD). The relationship between OP and COPD has been primarily studied in male patients, and few reports are available in postmenopausal women. Objective: The purpose of this study was to investigate the association between bone mineral density (BMD) and COPD in postmenopausal women. Methods: This cross-sectional study included 133 clinically stable female ex-smokers with confirmed COPD, and 31 age-matched “ex-smoker” female controls. We analyzed groups according to their airway obstruction category. BMD was measured on dual-energy X-ray absorptiometry images of the left femoral neck. Results: Patients with COPD had lower T-scores and higher prevalence of osteopenia/OP than the control group. In the COPD group, the airway obstruction category was significantly associated with the T-score after adjustment for confounders. Multivariate logistic regression analysis showed COPD was an independent marker for increased risk of osteopenia/OP in postmenopausal women. Conclusions: COPD and airway obstruction category were strongly related to BMD. Postmenopausal women with COPD, especially those with severe airway obstruction, had a higher prevalence rate and a higher risk of osteopenia and OP than female controls without COPD.
