Nowadays limited data are available about mechanisms of the resistance to COPD in part of heavy smokers. Components of the proteinase/antiproteinase, oxidant/antioxidant and kallikrein-kinin systems were identified in induced sputum by biochemical methods, biocidal properties of neutrophils were evaluated on the basis of the intracellular cation protein and myeloperoxidase concentration. In non-COPD heavy smokers higher levels of proteolysis inhibitors (α1-proteinasae inhibitor and α2-macroglobulin, acid-fast inhibitors) and lower levels of proteinases (elastase, trypsin-like proteinase) were detected. In these subjects the kallicrein-kinin system in induced sputum was less active,catalase was more active and the malondialdegyde concentration was lower. In the cytoplasm of neutrophils the were more inclusion granules with cation protein and myeloperoxidase in non-COPD subjects.
