Splenic epidermoid cyst

A 19 year old male presented with abdominal pain and left hypochondriac mass since one year. There was no history of trauma. Per abdomen revealed massive splenomegaly with smooth borders. Ultrasound examination showed three large splenic cysts with calcification noted in one of the cyst wall. With the clinical diagnosis of hydatid cyst, elective splenectomy was performed. Pneumovav was administered immediately after surgery. Grossly, 80% of the spleen was involved with three large cysts, biggest one ms 11x10cms (Figure 1). Microscopy showed cysts lined by stratified squamous epithelium and surrounding congested splenic tissue (Figure 2). A final diagnosis of splenic epidermoid cyst was made. The patient is doing well two years after surgery.
