Specifics of Aneurysm Treatment Distal Aneurysms

Abstract Distal intracranial aneurysms comprise only a small minority of intracranial aneurysms, yet carry a higher risk of morbidity, and should thus be treated aggressively. Distal intracranial aneurysms should be divided into true aneurysms (saccular or fusiform) and pseuodoaneurysms (mycotic, traumatic, dissecting, neoplastic), each with distinct treatment ramifications. Treatment can be undertaken by both endovascular and open surgical means, depending on the aneurysm etiology. When possible, true aneurysms should be treated with preservation of the parent vessels; however, pseudoaneurysms usually cannot be cured unless a normal vessel is reached on either side, making trapping with or without bypass or parent artery sacrifice, often feasible due to the distal location, a more successful method of treatment.
