Background: Dissecant aneurysms and ruptures affecting the thoracic aorta are hard to resolve. Endovascular repair of thoracic aorta aneurysms offers considerable potential and advantages over the surgical approach to treatment, partly because to surgical repair is associated with high mortality and morbidity rates. Objectives: To report on experiences using selfexpandable stents for the treatment of selected patients with aneurysms or dissections adversely affecting the thoracic aorta, describing the technique employed. M e t h o d s : S i x t e e n p a t i e n t s w e r e e v a l u a t e d retrospectively with dissection of the aorta and thoracic / abdominal aneurysms, and fourteen patients with dissecting aneurysm and two ruptured aneurysms. Six patients were female and ten male, between 66 and 87 years old, monitoring them in the intensive care unit after the procedure, with follow-up on an outpatient basis. Results: All the evaluated patients recovered successfully after the procedure with good clinical outcomes immediately after surgery and later during follow-up, except for: one patient who died in the immediate phase; one patient who died during the follow-up period for reasons not related to the treatment; and loss of contact with two patients during the follow-up period. Conclusions: Endovascular repair of aortic aneurysms shows considerable feasibility for selected patients. This may prove especially useful for patients with significant co-morbidities who are at high surgical risk,
