surgical operation (n 1). Patients’ demographic data, clinical symptoms, endoscopic findings, and CMV-viral load were reviewed. Results: CMV colitis was diagnosed in 30 patients. Seventeen (57%) were male. The mean age was 43 years (range, 17-83 years). All were immune compromised host. Twenty-two patients (73%) had AIDS with the mean absolute CD4 count of 50/ l (range, 4191/ l). Other immune-suppressed conditions included; prednisolone therapy (n 6; 20%) with the mean dosage of 22.5 mg/day (range, 10-40 mg/day), post chemotherapy (n 1; 3%) and other underlying diseases (n 1). All patients presented with various manifestations at the time of diagnosis. Diarrhea was the main manifestation (n 23; 77%) and seven were bloody (23%). Other clinical symptoms were fever (n 23; 77%) and abdominal pain (n 15; 50%). All except one underwent a lower endoscopy. Twenty-six patients (87%) had colon ulcers (multiple ulcers (n 21; 70%), single ulcer (n 4; 13%) and 2 ulcers (n 1; 3%)). The distribution of ulcers were pancolonic (n 14), left colon (n 7), right colon (n 3), isolated ulcer at IC valve (n 1) and isolated ulcer at terminal ileum (n 1). There were 5 patients with ulcers in the terminal ileum and 4 patients with ulcers at the ileocecal valve on top of the above-mentioned ulcers. PCR for CMV was performed in 24 patients. The mean viral load was 4.57 logC/ ml,(range, 2.60-5.41 logC/ml). Thirteen patients (43%) had CMV viral load more than 4 logC/ml. Extracolonic CMV infection were found in four (all were AIDS patients); CMV retinitis (n 3) and CMV pneumonitis (n 1). Conclusions: The main underlying of CMV colitis in our institution was AIDS with CD4 50/ l. The main presentations were diarrhea and fever. To diagnose CMV ileocolitis, colonoscopy with ileal intubation is recommended since terminal ileum involvement can be found in one fifth of the patients.
