One-stage breast reconstruction using the inferior dermal flap, implant, and free nipple graft.

The inferior dermal flap with implant and free nipple graft is a useful option for women with ptotic breasts who want immediate breast reconstruction. This one-stage procedure utilizes autologous tissue for an inferolateral dermal sling that can be sutured to the pectoralis major, which supports an implant and shapes the breast. Use of the ipsilateral nipple as a free nipple graft achieves an immediate color match to the contralateral nipple and has a far superior cosmetic outcome in comparison with that produced by postoperative areolar tattooing or delayed nipple reconstruction. Costs and potential morbidity of prosthetic mesh and prolonged operating times can be avoided. This method offers a safe, reliable and aesthetically acceptable outcome for women with larger, ptotic breasts.
