Domestic trends of hypospadias surgery

2016
Objective To evaluate the domestic trends of hypospadias surgery so as to provide rationales for improving the treatment of hypospadias. Methods Most domestic physicians were invited to answer questions through emails. The major contents included operative age, surgical procedures, preoperative treatment and postoperative complications. Then the answers were divided into several groups according to surgeon age and annual count of operations. One group was made up of low-volume surgeons ≤100 (50), high-volume surgeons >100(32) and surgeon aged ≤50(59) or >50(23) years. Results A total of 82/90 valid questionnaires were obtained. And 98.8% (81/82) of participants suggested that the ideal therapeutic age was 6-30 months for hypospadias. Meatal advancement & glanuloplasty (MAGPI) and tabularized incised plate (TIP) were reserved predominantly for hypospadias without chordee. And TIP, Duckett and two-stage were preferred for hypospadias with chordee. Surgeon age had no impact upon the choice of technique. Over 100 cases annually selected Duckett for repairing. And 48.8% (40/82) participants offered no treatment for penile curvature 35 degrees. Urethral fistula and stricture remained major postoperative complications. Conclusions The procedures of MAGPI, TIP, Duckett and two-stage are most commonly employed for hypospadias. Annual repairing frequency and surgeon age have no impact upon the choice of technique for hypospadias without chordee. Key words: Hypospadias; Questionnaires; Clinical Protocols
