The invention relates to a method of preparing medical polypropylene monofilaments with improved biocompatibility and discloses a novel method of preparing medical polypropylene monofilaments with improved biocompatibility. The method comprises the following steps of: by adopting medical-level polypropylene slices and pearl powder as materials, fusing and extruding out monofilament fine flow by adopting a novel screw extruder after kneading by a kneading machine; winding and forming to obtain the modified polypropylene monofilaments containing pearl powder after water-bath cooling, solidifying, stretching and heat-setting. The monofilaments has diameters of 0.07 mm-0.20 mm, fineness of 35 dtex-300 dtex, breaking strength of 3.0 cN/dtex-5.0 cN/dtex and elongation at break of 15%-30%. The pearl powder content is 1%-5%, and cytotoxicity is graded as grade 0 through an MTT (3-(4,5-Dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide) method test. The product has good stability and uniformity, various mechanical performances reaching the level of the common medical-level polypropylene monofilaments, and biocompatibility superior to the common medical-level PP (Polypropylene) monofilaments.
