Presentation of Case A four-day-old male infant was admitted to the hospital because of respiratory distress. The child had been born five weeks before term; the birth weight was 7 pounds, 9 ounces. Immediately after birth respiratory distress developed, and oxygen was given. Four days later x-ray examination revealed emphysema of the right lung. During the pregnancy the mother had had an excessive gain in weight and intermittent glycosuria; a glucose tolerance test performed during the pregnancy had been negative. Physical examination revealed a jaundiced child who became mildly cyanotic without supplemental oxygen. The trachea was shifted to the left. . . .
