Clinical analysis of pulmonary infection in hemodialysis patients

Objective To investigate the clinical setting and predisposing factors in hemodialysis(HD)patients with pulmonary infection.Methods 52 HD patients with pulmonary infection were analysed and many testing indexes were compared with those of contemporaneity 61 HD patients without infectious diseases.Results Not all of symptoms and signs of pulmonary infection in 52 HD patients matched with testing results by x-ray and laboratory.Among the 56 strains of germs isolated from the cultivation of 52 subjects′sputa,26 of them were Gram-negtive bacteria(46.4%),Gram-positive cocci 19(33.9%),anaerobes 2(3.6%)and fungus 9(16.1%);two kinds or more kinds of antibiotics were used in 53.8 percent patients.The ratio of diabetic mellitus,causing end-stage renal disease,levels of hemoglobin,albumin and plasm calcium concentration were significant between 52 HD group and 61 HD group.Higher ratio of diabetic mellitus,lower hemoglobin level and lower plasm calcium concentration were predisposing factors in HD patients with pulmonary infection by multiple linear regression.Conclusion Clinical charactors of pulmonary infection in 52 HD patients are ambiguity,the main infection cause in 52 HD group is gram-negtive bacteria,to minimize the bacteria drug resistance and the side effects of drugs,the clinical medication of antibacterials should be rationally guided by the results of bacteria susceptibility tests.Diabetic mellitus,hemoglobin level and plasm calcium concentration were predisposing factors in HD patients with pulmonary infection,therefore,these predisposing factors shoud be monitored to cure pulmonary infection.
