Segurança do paciente na administração da antibioticoterapia em UTI Adulto: Reconhecendo as condições geradoras de risco através do instrumento ASPAM

Objective: To recognize the conditions that generate risk for patient safety in an Adult Intensive Care Unit (ICU - A) in the process of antibiotic administration. Methodology A descriptive observational study conducted in an ICU - A, in Rio Grande do Sul. Seventeen nursing technicians participated in the study assisting patients in the data collection period from April to October 2020, through direct observation of antibiotic administration. The instrument ASPAM - Evaluation of Patient Safety in Drug Administration was used for data collection, which evaluated the nine domains of safe medication administration. The data were analyzed by means of descriptive statistics, with continuous variables represented by mean and standard deviation or median and interquartile range (IIQ) and categorical variables by relative and absolute frequency. Main results Low adherence to care was identified in the use of aseptic materials and techniques, and only six (35.3%) professionals performed this care every time in the preparation of antibiotics. Regarding the right register, only three (17.6%) professionals always registered the time just after the administration. As for the orientation carried out to the patient and/or companion about the drug administered, eleven (64.7%) professionals never oriented. Conclusions the conditions that generated risk to the safety of the patient in the administration of the antibiotic therapy were mainly directed to the low adherence of the cares for the right way, right register and correct orientation. It was thus identified the need for improvement of both assistance processes and team training.
