Afazija i hemodinamske promjene analizirane transkranijskom dopler sonografijom u bolesnika s moždanim udarom [Aphasia and hemodynamic changes analyzed by transcranial doppler sonography in patients with stroke]

The aim of the study was to determine whether there is a connection between the recovery from aphasia in patients with first ischemic stroke and hemodynamic changes in brain circulation analyzed by transcranial Doppler sonography (TCD). In-hospital patients with stroke and aphasia treated from the 1st April 2004 to 1st February 2008 were analyzed. The stroke burden was measured by National Institutes of Health Stroke Scale. Aphasia was assesed by Reinvang's Grunntest. Hemodynamic changes were measured by TCD on the third, seventh and ninetieth day of stroke occurence. Statistical analysis has been performed by using the STATISTIKA analytical system. Out of total 36 patients included (50% men; 50% women) there are 31 patients who survived. Non-parametric correlation analysis has proved a connection between Grunntest and mean velocities on the third day from the occurence of stroke in carotid syphone and middle cerebral artery (MCA) on the both sides. The examinees having no determined MCA occlusion (27/36; 75%) show better survival. The MCA occlusion was recoded with one fourth of examinees and global aphasia was characterized by a worse outcome. Therefore, this group of patients should be included in intensive rehabilitation program.
