Impacto de un programa de validación transversal en atención farmacéutica al paciente con insuficiencia renal

Objective: Description of pharmaceutical interventions performed on patients with kidney failure (KF) according to the cross-sectional validation (CV) methodology, and by means of a new computer software tool. Method: 7-month observational study in hospitalization and emergency services. We proposed CV as a complement to the traditional pharmacotherapeutic validation. For such a purpose, we designed a software tool that: 1. Integrates the hospital's computer systems; 2. Reports those patients with ClCr (Creatinine Clearance) <60 ml/min/1.73 m 2 and drugs that require a recommendation on KF; and 3. Sends those recommendations to the physician. Results: 2,535 patients showed a ClCr < 60 ml/min/1.73 m 2, 806 of them also had some drugs that required recommendation on KF. 295 interventions were performed: 48% accepted and 19% rejected (the remaining were classified as "not applicable"). The active principle and department with the most interventions were respectively levofloxacin and the Emergency Department. A pharmacist performed, on an average time of 26.7 minutes/day, the CV of the KF DRP (Drug Related Problem) in all patients. Conclusions: CV increases the efficiency in detecting and resolving DRPs, allows for an early detection in the Emergency Department and helps in the decision-making process
