Cardiac transplantation is the treatment of choice for end stages of heart diseases. Allowing the incidence of orthopedic complications such as osteonecrosis of the femoral head (2 to 3 %) due to the administration of inmunosuppresive agents, specially steroids it s dosages and time of use. We report the case of a 76 year old man with a survival time of 25 years post cardiac transplant, who suffer osteonecrosis of femoral head 12 years ago when was treated by drill perforations (diameter 9 mm) of the spongiosa of cervical and subchondral necrotic bone “Bone Decompression” Already evaluation with pain-function-image Harris Hip Score, show satisfactory results, 87/100 points, avoiding progression of the disease and arthroplasty procedure.
