The impact of repeat parathyroid scan in patients diagnosed with primary hyperparathyroidism

1941 Objectives Aim of the study was to evaluate for the impact of repeat Sestamibi parathyroid scan in patients with primary hyperparathyroidism for localizing parathyroid adenoma/hyperplasia. Methods 637 parathyroid scan reports and images were reviewed from September 1, 2007 through December 31, 2013. 477 parathyroid scans were performed on 429 patients diagnosed with primary hyperparathyroidism. Results There were 40 patients with multiple parathyroid (87) scans for parathyroid adenoma/hyperplasia localization. In fifteen patients initial scan did not localize any hyper functioning parathyroid tissue. On repeat scans, localization was successful in 9 (60%). Hyper functioning parathyroid tissue was localized in two on repeat scan; in four after a subsequent technetium 99m pertechnetate thyroid scan; in two after I-131 therapy for concomitant thyrotoxicosis and in one patient with concomitant localization was achieved with combined CT neck and parathyroid scan findings. Conclusions A repeat parathyroid scan may improve localization of parathyroid adenoma/hyperplasia in patients with primary hyperparathyroidism.
