The purpose of this study was to compare clinicomorphological characteristics and surgical treatment outcomes in women and men with lung cancer in order to specify contemporary approaches to the treatment and to improve outcomes. A total of 2,343 patients with morphologically verified non-small cell lung cancer receiving surgical treatment at the Thoracic Department, Clinical Oncology Research Institute, N. N. Blokhin RCRC during 1980 through 2006 were enrolled. Peripheral cancers were discovered in 242 (79.7%) of 310 women and in 772 (38%) of 2,033 men. Women with T2 and T3 disease demonstrated a more aggressive course of intrathoracic metastases. Overall frequency of lymphogenic metastasis in women was significantly higher if their tumors were ? 5 cm, and frequency of mediastinal lymph node involvement (N2-3) was higher with tumors > 3 cm (p
