STUDDING THE MOST IMPORTANT CAUSES OF HOSPITAL ERRORS EMERGENCE IN EDUCATIONAL-HOSPITAL CENTER OF KERMANSHAH PROVINCE DURING 2012

Introduction: Various elements play a role in complex systems such as hospital and health service providers and if an error or mistake accurse in one section of such system it will extend to other sections too. Therefore, we should use policies in such systems to minimize the possibility of any mistakes or errors. Based on existing statistics, many patients lose their health or lives in most developed health care centers annually, due to medical errors which creates many mental and physical issues for the patients, their families and also the community. Research method: Present study is applicable, descriptive - analytical and measure based. Kermanshah medical university specific and similar forms of error report were presented to all medical - educational centers from the beginning of the 1390 and were gathered monthly to gather the required data. Gathered data were analyzed by means of descriptive statistic methods (tables and diagrams) and using SPSS18 software. Findings: The distribution of error abundance based on the reason in all educational hospitals shows that the highest reason reported is related to personnel neglect with 547 reports (78.1%) and the minimum is related to hardware problems with 10 reports (1.4%). In relation to the source of errors reported in forms, 340 errors which includes 21.4 % of all errors were reported by anonymous individuals and the minimum were reported by hospitals' matron (1.1%) with eight reports. Conclusion: Identifying the type of errors and mistakes could be effective at decreasing them by health and treatment decision makers and drives attention and sensitivity toward this issue. From the other hand, by minimizing exposure obstacles, organizational errors and mistake will decrease. Employees report their mistakes when they feel safe and reporting their mistake has no sever consequences for them. It is proper if these centers managers and directors reconsider the error reporting system and organize non-punishing error reporting system so the employees are encouraged to report the mistakes and errors and managers should face the mistake based on system based procedures.
