Administrare minim invazivă de surfactant la nou-născutul cu greutate extrem de mică la naştere (ELBW) - prezentare de caz

Introduction. Minimally-invasive surfactant administration in spontaneously breathing neonates – tracheal instillation by feeding tube or angiocatheter – is an alternative that can avoid complications of tracheal intubation. Methods. The authors present the primary outcome of an extremely low birth weight premature who received prophylactic surfactant administration through Eutral Surfactant Kit catheter continued with non-invasive ventilation (CPAP) in Târgu-Mures Regional Center of Neonatal Intensive Care. Results. A female preterm neonate, with 26/27 weeks of gestational age, weighing 850 g at birth, Apgar score 6/1 minute, 8/5 min with alveolar recruitment in the delivery room and prophylactic surfactant administration through minimally invasive technique, had a favorable primary outcome in our unit: she did not require repeated surfactant administration or mechanical ventilation, specific complications were acceptable, medium term prognosis is good. Conclusion. We consider that the implementation of one protocol of minimally invasive surfactant administration in ELBW preterms is a challenge to establish a correct therapeutic attitude.
