Expansion of Intratumoral Hemorrhage in a Patient With Macroprolactinoma Treated With Cabergoline

A 25-year-old woman presented with amenorrhea and galactorrhea. Prolactin levels were elevated and magnetic resonance imaging showed a 10-mm pituitary adenoma with intratumoral hemorrhage. Because of failure of bromocriptine therapy, the patient was given cabergoline. During cabergoline treatment, she complained of severe headache, and visual field defects were demonstrated. Magnetic resonance imaging disclosed a significant enlargement of the intratumoral hemorrhage. The patient underwent a transsphenoidal removal of the pituitary mass. Three months after surgery, she conceived spontaneously. This case illustrates the possible aggravation of intraprolactinomal hemorrhage during cabergoline therapy
