Uso dos Serviços de Saúde: análise das redes de internações no Estado Rio de Janeiro, no ano de 2018

The use of health services can be explained by the supply side and on the demand side. Beyond that, the interaction between health services and users form networks of displacements that reflect the characteristics of these elements. The analysis of this information allows the survey of problems or right answers regarding the use, it can also aid the promotion of practices directed to the reduction of access barriers, besides contributing to the reformulation of the design of policies to reduce inequalities. Therefore, the objective of this study is to analyze the use of health services in the state of Rio de Janeiro, through the profile of hospitalizations and networks formed by the displacement of SUS users in search of care, in 2018. It is an exploratory analysis, which uses data from the Hospital Admission Authorization (AIH). These informations were analyzed from the optic of descriptive epidemiology and then it was possible to perform the profile of hospitalizations, local of residence and hospitalization. The networks formed by the source and destination flows of SUS users were analyzed through graph theory. In addition, calculations of the distances traveled and the duration of the displacements were performed. The state of Rio de Janeiro holds a very peculiar socio-economic spatial organization in the Brazilian case, marked by contrasts. In this sense, the occupation process and the geographic distribution of the population define the composition and configuration of the provision of services. The unequal distribution of health services in the networks, mostly with concentration of establishments in central and strategic municipalities of the State, suggests that the supply of services and their characteristics maintains a relevant influence on the use. Problems were found in the distribution of services and hospitalizations that can be reflected in the form of public policy management. In addition, profiles of hospitalizations and different services were observed in some networks, such as a larger number of philanthropic hospitals inland. The use of services studied in this work, through the analysis of the profile of hospitalizations and displacement networks in the state of Rio de Janeiro, reflects characteristics of users, cities and health policies that have been modified over time. These components are determinants of the use of health services. According to it, the analysis of networks becomes a tool that provides the analysis of public policy and also the use of services.
