Postoperative Analgeisa with Clonidine Release Patches in Combination with Local Infiltration of Levobuprivacaine in Patients of Anorectal Surgery with Essential Hypertension

Objective To investigate the effect of postoperative analgeisa with clonidine release patches in combination with local infiltration of levobuprivacaine in patients of anorectal surgery with essential hypertension. Methods Sixty patients underwent anorectal surgery under caudal anesthesia were randomly divided into two groups in according to different methods from Sept. 2007 to Aug. 2008. For postoperative analgesia,clonidine release patches in combination with local infiltration of levobupivacaine was given in groupⅠ,local infiltration with the mixture of lappaconitine and levobupivacaine was given in group Ⅱ. The levobupivacaine concentration was 0.125 percent,diluted with saline, the total amount of 15ml. Group Ⅱ was diluted by adding liquid lappaconitine 6mg. Recorded of the patients postoperative analegesia 2,4,8,10 hours Ramsay and VAS pain scores,at the same time recorded of adverse reactions and treatmeant measures to moniter plasma glu- cose at the admission,8,24,48 hours and recorded HR,MAP. Results VAS pain scores were significantly lower in group Ⅰthan those in group Ⅱ(P0.05)and Ramay sedation score in group Ⅰ was higher than those in group Ⅱ(P0.05)during postoperative analgesia,and that there was no over-sedation group. HR and MAP after operation in group Ⅰwere markedly decreased compared with admission(P0.01). The concentration of plasma glucose after operation in group Ⅰlower than group Ⅱ(P0.05). The rate of adverse reaction in two groups were not significantly different. Conclusion Clonidine transdermal patches can enhance the analgesic effect of levobupivacaine,alleviate pain caused by adverse reactions with stable hemodynamics in patients with essential hypertension.
