Alar ligament injury can occur from its attachment to different lengths, when the head is subjected to a hyperflexion or a contralateralrotation, involving sudden and high power forces; Usually ending in a rupture of the ligaments in their weakest site, the union with the occipital condyle. In this process the speed of stretching is the main determinant of injury. In the literature, very few reports of rupture of this ligament have been reported, and most are associated with trauma from traffic accidents or sporting events, being the first case described with a mechanism of trauma associated with a firearm.
