Pyrimethamine-induced transient leukoencephalopathy: similarity to methotrexate encephalopathy

We describe the case of a 29-year-old female with ocular toxoplasmosis who presented with a consciousness disturbance. Diffusion weighted images (DWI) revealed the presence of multiple high-intensity areas in the bilateral cerebral white matter (Fig. 1a). Correspondingly, on the apparent diffusion coefficient (ADC) map, ADC values were found to decrease (Fig. 1b), whereas fluid-attenuated inversion recovery (FLAIR) images showed no abnormal intensities (Fig. 1c).
