The evolutional research of septic cardiomyopathy

Sepsis is defined by consensus as a life-threatening organ dysfunction caused by a dysregulated host response to infection. Septic cardiomyopathy with an impairment of cardiac functionis is one of the complications of sepsis, however, the myocardial dysfunction is reversible. The physiopathologic mechanism inflammatory invovled in cytokines, immunoreaction, dysregulated intracellular calcium transporter protein, mitochondrial dysfunction, structural abnormality of extracellular histones and so on. To date, there isn′t a standard to diagnose and therapy septic cardiomyopathy. In spite of hsTnT, NT-proBNP, miRNA-497 and miRNA-21-3p can be used as myocardial markers for diagnosing septic cardiomyopathy,but they lack specificities. Recently, new myocardial markers such as lactoferrin and cluster differentiation15 which might be used for diagnosing septic cardiomyopathy have been reported. Currently, the treatment of septic cardiomyopathy is not standardized in clinical practice. In addition to conventional treatment methods such as IAPB and ECMO,the research and development of targeted molecules such as Pep2.5, caspase-11 and high mobility group box-1 protein have an important clinical application prospect. Therefore, it has a great clinical significance to strengthen the awarness on diagnosis and treatment for septic cardiomyopathy and reduce the mortality of septic cardiomyopathy through putting insight into the pathogenesis of septic cardiomyopathy.
