Key content Monoamniotic twins are believed to account for 1–5% of all monozygotic conceptions, with an estimated annual incidence of 30–150 pregnancies in the UK. High perinatal loss rates in monoamniotic twins have been attributed mainly to umbilical cord entanglement, inter‐twin transfusion syndrome, discordant fetal abnormality or fetal growth restriction. Management of monoamniotic twin pregnancy is aimed at preventing antenatal fetal death and optimising timing of delivery. Despite the paucity of robust data on the incidence and causes of perinatal loss, elective delivery at 32–34 weeks of gestation has been proposed. Most obstetric units use caesarean birth as the preferred mode of delivery for monoamniotic twins to prevent intrapartum cord complications. Learning objectives To understand the aetiology, incidence and diagnosis of monoamnionicity. To be able to identify the type and prevalence of complications in monoamniotic twins. To be aware of the current evidence on the antenatal management and optimal timing of delivery of monoamniotic twin pregnancy. Ethical issues Can we justify elective preterm delivery and its subsequent complications without a robust evidence base demonstrating a reduction in perinatal morbidity and mortality?

Conjectures about Justification

Abstract This chapter explores justification as a suitable object for modernizing uncertainty. It examines how people justify practical proposals and predictions, particularly cooperative justifications. The conjectures of justification, such as its role, supply, demand, and routines of organizations, go well beyond Frank Knight’s thesis about uncertainty and profit. The roles and challenges of justification revolve around agreements, authoritarian alternatives, and competition. The chapter provides an overview of the supply and demand for justification, including the notion of evidentiary weights and the role of cultural and technological influences. Meanwhile, routines that organizations use to evaluate and justify claims include organizational specialization, bias against uniqueness, qualifications, and functional reasonableness.

Calculating Capitalists: VCs and Angel Investors

Abstract This chapter examines the significance of professional VCs and angel investors. It explains that professional VCs and angel investors became prominent long-tail investors who aim for a few hits to make up for many misses. In the standard view, thorough evaluations allow VCs and angels to bridge the financing gap for new ventures that is largely due to information asymmetry. Investors rely on due diligence and ongoing oversight to control entrepreneurs’ honest mistakes and misjudgments. Moreover, stricter routines influence VCs’ investment choices, which references how strictness limits tolerances for irreducible uncertainty while encouraging specialization in larger, more complex initiatives.

Abstract Uncertainty and Enterprise examines the practical ways to deal with everyday uncertainties in the field of entrepreneurship. It presents an intuitive framework for coping with doubts about honest mistakes and sincere disagreements in correlation with the work of the economist Frank Knight and other great twentieth-century thinkers. Regardless of the perspectives of textbook economics, standard theories, and behavioralists, uncertainty is banished to the unmanageable world of unknown unknowns, and resolute action requires confidence in individual and collective choices. Uncertainty and Enterprise provides an overview of the interplay between startups and giants. It also discusses how seemingly bureaucratic procedures encourage giants to undertake complex high-stakes initiatives.
