<i>In Vitro</i> Evaluation of Cytotoxicity Induced by Indoxacarb and its Amelioration with <i>Cassia fistula</i> Bark Extract

The primary objective of the study is to evaluate the cytotoxic effect and biochemical alteration on exposure to the Indoxacarb (IDC) in the Madin-Darby Bovine Kidney (MDBK) cell line and its amelioration by Ethanolic Bark Extract of Cassia fistula (EBECF). The MTT assay was used to study the cell viability and determine the IC50 value. The cells were further treated with IC25, IC12.5 and IC6.25 values of IDC alone and in combination with EBECF for 24, 48 and 72 hours. IDC-induced cytotoxicity was evident by cellular morphological changes such as rounding of cells, cellular swelling, blebs formation, nuclear degradation, intracytoplasmic changes and loss of intracytoplasmic contents. The biochemical alteration was observed as a significant increase (p<0.05) in the levels of Lipid Peroxidation (LPO) and cytotoxicity marker enzymes namely Lactate Dehydrogenase (LDH), Alkaline Phosphatase (ALKP), Acid Phosphatase (ACP) and Creatine Kinase (CK). These changes were dose-dependent in IDC-treated groups. EBECF intervention in IDC-treated cells ameliorated the toxicity against lower concentrations. Thus, the present study suggested that C. fistula bark has the potential to protect the cells from IDC-mediated toxicity to a limited extent.

Burden of diabetes has escalated worldwide and global death rate has doubled in last two decades.About 88 million people are living with diabetes in Southeast Asian countries and it is expected to shoot up rapidly because of changing life style and longevity of life.Unfortunately, diabetes is not well managed in these countries because of several factors but mainly due to poor doctor-to-population ratio.Recently, World Health Organization has also advised accessible, affordable and quality diabetes care in Southeast Asian countries through extended role of nurses in diabetes care.The model of independent nurse-led diabetes clinic and advanced diabetes nurse practitioner for management of diabetes has been successful in the western world.This article highlights the scope, feasibility, challenges, barriers and facilitators in implementation of nurse-led diabetes clinics (NLDC) in Southeast Asian countries.

Awareness and perception regarding tuberculosis among patients and their relatives attending a tertiary care hospital in Uttarakhand: A hospital-based exploratory survey

Awareness about disease among tuberculosis (TB) patients plays a crucial role toward successfully achieving targets for control, prevention, and their relatives treatment adherence and is not well studied or documented. This study sought to explore the awareness and perceptions of TB patients in a tertiary care centre in northern India.This was an exploratory study conducted between January and December 2016 among 1,000 pulmonary TB patients and their relatives. Structured and validated interview schedule was used to assess participants knowledge and perception regarding TB, which comprised of 41 questions. Ethical clearance was taken and written informed consent was obtained from each study participants. Data analysis was done using SPSS 22.0 version.A total of 1,000 study participants (mean age 40.2 ± 9.6 years, females 51%) were enrolled. More than two-third of the study participants were from Uttarakhand. Study participants had highest knowledge score (61.85%) regarding sign and symptoms, followed by scores in the aspect of prevention and treatment of TB (52.7%). However, a lower proportion (51.5%) knew about its causation. Overall knowledge score was 54.8%. Around half of the subjects (49.7%) disagreed that TB is a major health problem.Regardless of non-satisfactory knowledge of participants, their perception regarding TB was better. As to the associated factors, we found that participants' knowledge had significant association with religion, educational status, occupation, family income per month, type of family, and source of health information. Although there was insignificant difference between family monthly incomes, source of health information and perception regarding TB.

Acquaintance, attitude, practices and challenges of palliative oral care among healthcare professionals: A cross-sectional survey at a tertiary healthcare institute in India

Introduction: To assess knowledge, experiences, perceptions and barriers of healthcare professionals regarding oral palliative care. Material and methods: The study involved 150 participants working at a tertiary healthcare institute in India. The data collection tool was pretested and self-administered with sections on demographics, knowledge, attitude and practices, patient referral, perspectives, and barriers to oral palliative care. Results: The majority of participants 142 (94.6%) believed that palliative care patients need oral care, 85 (68.6%) participants had treated palliative care patients with dental problems. However, 60 (40%) had not received formal training for assessment and referral of patients with oral problems; 95 (63.4%) had never used tools to assess oral conditions of palliative care patients. According to 69 (46%), the best method to maintain oral hygiene is rinsing with saline and 81 (54%) expected physicians to be responsible for oral care of palliative care patients. The main challenges in providing oral care were lack of proper guidelines listed by 117 (78%) and lack of formal training indicated by 60 (40%) respondents. Conclusions: This study highlighted the need for effective assessment of the mouth and appropriate oral care. Training of healthcare professionals, educating families and patients with oral palliative care are necessary to effectively manage oral symptoms. An appropriate patient follow-up and care delivery system should be structured at comprehensive cancer centres, which can improve the quality of life and compliance of patients. There is a need for the development of assessment tools and referral practices for providing relief, comfort and consolation to patients and families.

Noble mental health improves self-assurance and self-confidence and enables a person to love and motivate other people, daily life and surroundings.In this era of globalization, to maintain the mental health of society, it is necessary to adopt technological advancement and grow various mental health faculties.Digital India initiative of Government of India (GoI) for healthcare is taking a momentum through the new guidelines, policies and regulations.Use of EHR and EMR has been an important aspect to manage patient health information.NMHS discovered that approximately 80% of individuals affected had not received any type of care after the start of their disorder.The ill-health associated with these illnesses were severe in nearly 0.7% to 28.2% of individuals along with the significant socioeconomic effect on individuals who are affected.As its well-known psychiatric treatment is last longer as per the patient's prognosis and maintaining psychiatric patients record for long years is a crucial task by the patient's self and his caregivers.EHR serves as a well-documented record for patient's official treatment records & can be used for shielding malpractice, regulatory action or ethics complaint.MoHFW, GoI took national membership of SNOMED International and made SNOMED CT clinical terminology offered free-for-use in India.National Release Centre (NRC) for SNOMED CT was set up for pervasive acceptance and support of SNOMED CT in the nation.We may suggest that eHealth and EHR standard enabled clinical applications can improve rendering mental health services to patients and society.
