FOR many past summers, a disease entity characterized by fever with vesicular or ulcerated throat lesions and chiefly affecting children has been prevalent in the District of Columbia and its environs. During the summer of 1950 it became widely disseminated, assuming epidemic proportions during July and August. There was general agreement among the physicians practicing in these areas that the disease made its appearance with some regularity each summer, was highly contagious and usually ran a benign, self-limited course. Aureomycin in the Treatment of Pneumonia in Infants and Children

AUREOMYCIN is an antibiotic derived from a strain of Streptomyces aureofaciens. The drug has been found to have a wide antibacterial spectrum against gram-positive and gram-negative organisms. In addition, it has been found to be effective against rickettsia as well as certain viruses, chiefly of the lymphogranuloma-venereum-psittacosis group.1 In man, therapeutic doses up to 60 mg. per kilogram of body weight per day given orally have not been associated with toxic manifestations except for occasional nausea, vomiting and diarrhea. PLACENTAL TRANSMISSION OF CHLORAMPHENICOL (CHLOROMYCETIN®)

It has been shown that both streptomycin and penicillin are able to traverse the placental barrier and appear in the fetal circulation in therapeutic concentration. An experiment was carried out to determine whether chloramphenicol (chloromycetin®) similarly passes through the placenta. An oral dose of 1 Gm. of the drug was given to 8 patients and 2 Gm. was administered to 3 patients in active labor approximately one to five hours before delivery. Blood for chloramphenicol assays was obtained from the mother and cord at the time of delivery. In 1 instance, (case 11) the mother delivered twins and chloramphenicol assays were obtained on both infants. The serums were then tested for chloramphenicol content with a new chemical assay method developed in this laboratory by Bessman and Stevens.¹This method has proved to be more accurate than the bioassay method and considerably simpler to perform. THE susceptibility of various strains of salmonella in vitro to streptomycin has been adequately demonstrated by West et al.1 However, the clinical efficacy of this antibiotic in salmonella enteritis has not been too extensively investigated, and relatively few cases have been reported thus far among children. Seligmann et al.2 used streptomycin in 5 cases of enteritis due to Salmonella typhimurium in the newborn period and observed only a transitory sterilization of the bowel, and in all 5 cases the pathogenic organism reappeared in the stool, usually within forty-eight hours after the drug had been discontinued. THE role of penicillin administered by mouth has been reported on by several investigators,1 2 3 4 5 6 and the conclusion seems warranted that if the drug is given in sufficiently large doses, cures may be expected in susceptible infections. CHLOROMYCETIN is an antibiotic derived from a strain of Streptomyces venezuelae. Its antimicrobial spectrum is broad and embraces the following organisms: rickettsia, certain viruses of the psittacosis group, gram-positive cocci (including the pneumococcus, streptococcus and staphylococcus) and certain gram-negative organisms including Eberthella typhosa, shigella and several of the coliform group.1 The drug is well tolerated orally, and few toxic reactions have followed its use in therapeutic doses. Chloromycetin is readily absorbed after oral administration, and blood levels of 10 to 40 mg. per 100 cc. are readily obtained when 50 to 100 mg. per kilogram of body weight . . .

The Effect of Aureomycin and Chloramphenicol on the Fungal and Bacterial Flora of Children

SEVERAL reports in the literature have suggested that chloramphenicol and aureomycin commonly have undesirable side effects, especially involving the mucous membranes.1 2 3 These include glossitis, stomatitis, sore throat, proctitis, pruritus vulvae and and, nausea, vomiting, diarrhea, bronchomoniliasis, mycotic endocarditis and meningitis, and the sprue syndrome.4 5 6 7 8 Oral manifestations alone have been recorded in 6 to 20 per cent of patients who received aureomycin or chloramphenicol.1 Nearly all the patients in whom these complications occurred have been adults who had received relatively long-term therapy.The explanations for these phenomena vary. SALMONELLA infections have shown a singular refractoriness to chemotherapeutic and antibiotic agents thus far. The inability of sulfonamides to affect favorably the course of the disease is now well established.1 During the past year, 8 cases of salmonella enteritis were treated at Children's Hospital, Washington, D. C, with streptomycin in large doses.2 Only 1 patient was rendered permanently free of salmonella; in the other 7 cases, the organism reappeared in the stools after discontinuation of therapy.
