PREVALENCE AND ANTIMICROBIAL RESISTANCE IN ENTEROCOCCUS SPECIES

Objectives: The study was conducted to determine the prevalence of vancomycin-resistant Enterococci (VRE) isolated from various clinical samples received from the indoor patients of all age groups admitted in Government Medical College and Hospital, Amritsar. Methods: A prospective cross-sectional study was conducted in the Department of Microbiology, Government Medical College, Amritsar, for a period of 4 years (July 1, 2018–June 30, 2022). All the samples (pus, urine, blood, body fluids, sputum, etc.) received from the indoor patients of all age groups admitted in Government Medical College and Hospital, Amritsar, were included in the study. Results: During the study period of 4 years, among the culture positive samples, 1815 (6.62%) isolates were identified as Enterococcus species. Among 1815 isolates, 1089 isolates were Enterococcus faecalis (60%) and 726 were Enterococcus faecium (40%). Both E. faecalis and E. faecium isolates showed the maximum resistance to ciprofloxacin while linezolid, teicoplanin, and quinupristin/dalfopristin showed the maximum sensitivity. Conclusion: Our study reports the prevalence of Enterococci isolates as well of VRE isolates. To reduce the VRE prevalence worldwide, appropriate use of antibiotics according to antimicrobial susceptibility testing should be encouraged. Efforts should be made to reduce the transmission of VRE isolates.

Detection of Inducible clindamycin resistance in nasal carriers of Staphylococcus aureus among healthcare workers

Staphylococcus aureus () is recognized as one of the most common microrganisms causing nosocomial and community-acquired infections. Nasal carriage of Staphylococcus aureus is becoming an increasing problem among healthcare workers and in the healthy community individuals. General populations with persistent Staphylococcus aureus nasal carriage rates at 10% to 20%, and up to 50% are intermittent carriers. Furthermore, carrier levels of 25% have been reported among healthcare workers(HCW’s). Strains with inducible clindamycin resistance (ICR) are difficult to detect in the routine laboratory as they appear to be resistant to erythromycin and sensitive to clindamycin in vitro when not placed adjacent to each other. In such cases, in vivo treatment with clindamycin may select constitutive erm mutants leading to clinical therapeutic failure. 100 nasal swabs samples were collected during January 2019 to November 2019. Isolates were identified using standard microbioloical procedures and MRSA determined by the disk diffusion method. The D-test was performed for detection of Inducible clindamycin resistance isolates with Clinical Laboratory Standard Institute guidelines. : Out of 18 isolated, 33.3 were MRSA. Of 18 isolates tested for ICR by D-test, 6 (33.3%) yielded inducible resistance. Inducible clindamycin resistance was found to be significantly higher among MRSA than MSSA isolates. Beside personal hygiene practices of healthcare workers, regular implementation of infection control practices, including screening of nasal carriages and microbial flora in our hospital are necessary to prevent spread of MRSA. It is also advisable to perform routine nasal decolonization of healthcare workers using mupirocin ointment to prevent transmission of these organisms.

Fusarium infections are important problem worldwide, cause a broad spectrum of infections in human including superficial infections as well as locally invasive and disseminated infections. We report a rare case of perinephric abscess caused by Fusarium chlamydosporum in a child who had a recent episode of pyelonephritis. This case illustrates the ever increasing spectrum of rare but offending pathogenic fungi in an immunocompetent host. Fungal infections should always be suspected in patients having one or the other underlying risk factor or who are unresponding to antibacterial therapy. Early diagnosis of infection with a specific pathogen may lead to changes in antifungal therapy and may be critical for an improved outcome

Investigation of clinical parameters associated with COVID-19 in pregnant women

Background: Pregnancy is an immune-compromised condition; encompass physiological changes that make the women more vulnerable to infections such as COVID-19. The major concern is about the intrauterine transmission resulting in fetal and neonatal outcomes; therefore, this group of population requires more attention pertaining prevention, management and treatment. Thus, the main aim of the study is to analyze clinical characteristics among pregnant women.Methods: The study was conducted on 17092 samples of pregnant adults whose nasopharyngeal and/or oropharyngeal swab samples were received from various districts of Punjab (Amritsar, Gurdaspur, Hoshiarpur, Kapurthala, Tarn Taran and Pathankot) from the period of March 2020 to December 2020. Confirmation of SAS-CoV-2 was done by RT-PCR.Results: Out of total samples (17092), 374 (2.19%) females exhibited positive results for SARS-CoV-2, and mean age of the pregnant females was 26.37 years. Analysis was performed on 210 (Positive) samples as cases and 210 age matched control females (SARS-CoV-2 negative). Analysis performed demonstrated that SpO2 <96% and co-morbidities such as diabetes and hypertension showed significant association and are common factors in COVID-19. None of the case showed any neonatal and maternal demise.Conclusions: In the present study, saturation level of oxygen (<96%), diabetes and hypertension was found to be significant. All the pregnant women have similar symptoms as that of non pregnant adults.

Emerging Antimicrobial Resistance in Staphylococcus and Spectrum of Oral Oxazolidinone Linezolid in Multidrug Resistant Staphylococcus in a Tertiary Care Hospital

Introduction: Staphylococcus species is an asymptomatic colonizer which has exhibited increasing resistance to various antimicrobial agents in the recent times.Although, the organism was naturally susceptible to all the antimicrobial agents, it has acquired multi drug resistance via various mechanisms.In view of rising prevalence antimicrobial resistance, this study was undertaken to study the prevalence of methicillin resistance and the emergence of linezolid resistance amongst the Staphylococcal isolates obtained from various clinical samples in a tertiary care hospital. Material and Methods:This study was conducted from July 2017 to December 2018 in a tertiary care hospital.Clinical specimens were processed, and Staphylococcal isolates were identified using standard microbiological techniques.Antimicrobial resistance pattern of all the Staphylococcal isolates was determined and interpreted as per the latest CLSI guidelines.Results: A total of 989 Staphylococcal isolates were obtained amongst which 498 (49.44%) were found to be methicillin resistant.Highest antimicrobial resistance was observed to beta lactam antibiotics followed by azithromycin and fluoroquinolones.26 (2.628%) strains of Staphylococcal species were found to be resistant to linezolid while they retained sensitivity to Vancomycin. Conclusion:The emergence of drug resistance in various microorganisms has been a cause of global concern.Rising trend of resistance has been observed to methicillin and linezolid especially in indoor patients of the Intensive care units.This study highlights the high prevalence of Methicillin resistance in both Staphylococcus aureus and Coagulase negative Staphylococcus species in a tertiary care hospital in Amritsar, India.Although, linezolid resistance is emerging at a slow pace, adequate measures must be undertaken to preserve the therapeutic armoury.

Detection and Quantification of Cytomegalovirus in Immunocompromised Patients

Introduction: CMV disease ranges from asymptomatic infection in immunocompetent people to severe and lifethreatening infections in neonates and immunocompromised patients.The present study was undertaken to study the seroprevalence of CMV and use of Real-time PCR in detection and quantification of CMV in immunocompromised patients.Material and Methods: A total of 57 samples were tested for CMV IgM and CMV IgG by ELISA technique.The detection and quantification of CMV DNA was done by real time PCR.Results: Out of 57samples, 6 (10.53%) tested positive for CMV IgM, 50 (87.72%)were positive for CMV IgG and 18 (31.58%)were positive for CMV DNA by PCR.Among 18 PCR Positive cases, 6 (33.334%) cases showed viral load of 1x10 3 IU/ml.4 (22.22%)cases each showed viral load of 1x10 4 IU/ml, 1x10 5 IU/ml, 1x10 6 IU/ml respectively.There is no statistical association of age, gender and geographical distribution with CMV IgM, CMV IgG and CMV DNA positivity.(P>0.05). Conclusion:This study demonstrated that Real-time PCR is more reliable test than serological ELISA test in the diagnosis of cytomegalovirus as Real-time PCR detects and quantifies viral DNA which is useful in predicting the patient's risk for disease and monitoring the effect of antiviral therapy.Good hygiene and infection control practices are advised to prevent CMV transmission.

Introduction: Antimicrobial resistance is a cause of global concern as resistance is emerging enormously in hospital and community settings.The occurrence of resistance to cephalosporins due to production of Extended Spectrum Beta-Lactamases is known worldwide.Hence, this study was undertaken to detect the prevalence and antimicrobial resistance pattern of ESBL-producing gram-negative bacteria isolated from various clinical samples received from the indoor patients of a tertiary care hospital.Material and Methods: Clinical specimens received from the patients admitted in Guru Nanak Dev Hospital, Amritsar from January 1, 2018 to June 30, 2018 were included in the study.The samples were processed based on standard microbiological techniques.ESBL screening and confirmation were done based upon CLSI guidelines.Antimicrobial resistance pattern of ESBL producing gram negative bacteria was determined.Result:-A total of 8147 samples were received out of which 1061(13.02%) gram negative bacteria were isolated.227 (21.97%) of the gram negative isolates were positive on screening and 107 (10.08%) were confirmed to be ESBL producers phenotypically.Maximum antimicrobial resistance was observed to ciprofloxacin and amikacin.All the isolates were sensitive to sulbactam ceftriaxone and imipenem. Conclusion:The present study highlights the prevalence of ESBL-producing gram negative bacterial isolates in a tertiary care hospital in Amritsar, Punjab.Measures such as the establishment of antimicrobial stewardship activities, monitoring surveillance and infection control programmes, emphasizing on effective hand hygiene practices together with coherent antibiotic policies should be enforced in the hospitals to arrest the spread of ESBLs.
