IMPACT OF COVID 19 ON ENVIRONMENT AND NATURAL RESOURCES: AN OBSERVATIONAL STUDY

The COVID-19 pandemic, also known as the coronavirus pandemic, is an ongoing global pandemic of coronavirus disease 2019 (COVID 19), caused by severe acute respiratory syndrome coronavirus 2(SARS CoV 2). COVID-19 needs to be seen beyond just the number of cases or deaths, especially in the context of its impact on air pollution, decline in road traffic accidents, improvement in air and water quality, decline in respiratory infection, nosocomial infections and pediatric chest infections, though some negative effect can be seen on the physical activity and hospital related transmission of COVID-19 to healthcare workers. An invisible virus has shown that it is possible that tangible policies can make world a better place to live in.

"Quick Cycle" neoadjuvant chemotherapy in squamous cell carcinoma of cervix

Objective: To evaluate the efficacy and safety of a 'Quick cycle' neoadjuvant chemotherapeutic regime in squamous cell carcinoma (SCC) of cervix and monitor the response to chemotherapy using Squamous Cell Carcinoma Antigen (SCC-Ag) as tumor marker.
Materials and Methods: Thirty patients with SCC of cervix (Stage I-IV) entered the study to receive three courses of multiagent neoadjuvant chemotherapy (vincristine, cisplatin and bleomycin) in a weekly regime. SCC-Ag was evaluated in these patients' pre and post-chemotherapy.
Results: Patients with stage IB2 had a complete response and two of seven patients with stage IIB became operable. The overall response rate was 47.8% and complete response rate was 8.7%. Decrease in tumor volume post-chemotherapy was significant (P<0.002). Toxicity including myelosuppression was minimal. A statistically significant decrease in SCC-Ag (stage II and III) was seen post-chemotherapy (P <0.04 and 0.005, respectively).
Conclusion: The weekly chemotherapeutic regime was found to be safe and effective and SCC-Ag is a useful tumor marker for monitoring response to chemotherapy.

A rational approach to manage surgical procedures in COVID Era – A perspective based on experience in a private referral hospital

COVID-19 disease has imposed challenges in caring for non-Covid elective surgical patients. As elective surgeries become essential, we propose to evaluate our approach and outcomes of surgical procedures performed during the initial period of COVID-19 pandemic so as to provide a road-map for safer approach.We retrospectively evaluated outcomes in patients who underwent essential elective and emergency surgeries during the 5-week period between April 18, 2020 and May 28, 2020. All patients were screened at the front desk on their arrival to identify possible exposure to SARS- CoV-2. Nasopharyngeal swab of patients requiring hospital admission was tested for COVID-19 by quantitative RT-PCR. Patients needing essential elective surgery were taken up for surgery if they tested negative for COVID-19. Emergency procedures were undertaken in a demarcated theatre for COVID after taking level-3 protection without delay. The clinical data was reviewed and analysed.A total of 764 surgical procedures were conducted, of which 70.7% were elective essential surgeries, with 95.4% of these patients being discharged in stable healthy condition. Approximately 23% of the elective and 26% of the emergency surgeries was categorised in the surgical difficulty category III and majority of these were performed under general anesthesia. Postoperative mortality was 1.04%, but the overall mortality rate was approximately 2.5%. Only two patients (0.3%) tested positive for COVID-19 in our series.A robust preoperative screening and testing can enable safe scheduling of essential elective surgeries.

Extraction & assay of fibronectin from stored plasma.

Fibronectin was extracted from 20 units of 3 days old plasma by the heparin cold precipitation technique using 15 or 30 units of sodium heparin per ml of plasma. Immunoelectrophoresis of the extracted fibronectin showed it to be 131.37 +/- 24.73 times concentrated over fibronectin levels in standard plasma, with a mean recovery of 73.99 +/- 18.40 per cent. Corresponding figures by ELISA were 118.95 +/- 27.99 (concentration) and 73.99 +/- 18.40 (per cent).

Comparison of levobupivacaine alone versus levobupivacaine with ketamine in subcutaneous infiltration for postoperative analgesia in lower segment cesarean section

Context: Local anesthetic wound infiltration is employed as a part of multimodal analgesia to reduce opiate consumption and pain after lower segment cesarean section (LSCS). Additional blockade of pain pathway at spinal level by ketamine prolongs the duration of analgesia. Aims: To compare analgesic efficacy of subcutaneous wound infiltration of levobupivacaine or levobupivacaine plus ketamine following LSCS. Material and Methods: Randomized double blind study was conducted on 60 parturients undergoing LSCS under spinal anesthesia. Group A received surgical wound infiltration with 0.5% levobupivacaine 2 mg/kg body weight and Group B parturients were infiltrated with 0.5% levobupivacaine plus ketamine 1 mg/kg body weight diluted with normal saline. Postoperative pain scores, time to first rescue analgesia (FRA), hemodynamic parameters, and total opioid analgesic consumption were assessed. Results: Pain free period and time to FRA was 1.5 hours later in group B, which also had reduced mean VAS scores. In addition, the overall pain scores and total opioid consumption were significantly less (P = 0.003) in Group B. Only 50% in Group B and 97% parturients in Group A needed rescue analgesia. Patient satisfaction score was statistically superior in Group B (P = 0.009). Incidence of nausea and vomiting was comparable between the groups (P = 0.554). Conclusions: Addition of ketamine to levobupivacaine for surgical wound infiltration prolongs the analgesia duration, improves patient satisfaction, and decreases 24-hour opioid consumption.
