LOW FIBRINOLYTIC ACTIVITY IN THE WALLS OF VEINS IN PATIENTS WITH THROMBOSIS

Abstract The fibrinolytic activity of biopsy specimens of veins from 58 patients with superficial thrombophlebitis and/or deep venous thrombosis of the limbs, from nine healthy volunteers and from ten patients about to undergo biliary surgery has been studied by a modification of the "fibrinolysis autography technique" of Todd. The findings appeared to warrant the following conclusions. 1 The fibrinolytic activity of unaffected veins in the legs of patients with thrombosis and thrombophlebitis is significantly lower than that of veins in normals. 1 In veins occluded by compact thrombi the fibrinolytic activity is negligible, but reappears when the thrombi become canalized. It appears that low fibrinolytic activity of the vessel walls predisposes to thrombosis.

Antihemophilic factor A-von Willebrand factor (AHF-vWF) is a protein involved in the first phase of blood coagulation and in the mechanism of platelet adhesion. The localization of this protein has been studied in the anterior segment of fetal and adult human eye by a direct immunofluorescence method. It was found that AHF-vWF is present in the wall of aqueous outflow pathways but not in that of Schlemm canal. As it has fibrinolytic activity in the endothelium of Schlemm canal, this structure can be expected to be especially well protected against fibrin deposition and occlusion by platelets and thrombi. The ciliary processes of the adult eye had no AHF-vWF activity. Instead, fluorescein isothiocyanate-fluorescence could be seen in the vessel wall of the ciliary processes of the fetal eye. This difference may be connected with the function of aqueous secretion.

Localization of Fibrinolytic Activity in the Developing Rat Eye

The histological localization of fibrinolytic activity in the developing eye of rat was studied by means of the histochemical fibrin slide technique. The observation that fibrinolytic activity is related to small vessels provided a means of following the developing vascularization of the eye. The hyaloid vascular system was found fully developed, together with the tunica vasculosa lentis, at the 18th day of intrauterine life. Involution of these vessels begins between the third and the seventh day after birth. The retinal vessels begin to develop between the 16th and the 18th day of intrauterine life. The choroid acquires fibrinolytically active vessels in the second week of life. At this time also the ciliary body becomes vascularized. The patterns of appearance and disappearance of sites of fibrinolytic activity observed during the development of the eye are related to the functional morphology of the involved tissues.
