Morphological Study of Human Fetal Lung with Respect to Gestational Age in the Tertiary Care Center of Kumaon Region of Uttarakhand

Introduction: The lung fissures are helpful in the movement of lobes in relation to one another. Variation in the presence or absence of fissures indicates about the pattern of development of the lung. The study was done to observe the appearance of lung fissures in human fetal lungs in relation to the gestational age. Materials and Methods: This cross-sectional study was conducted in the Department of Anatomy on 32 pairs of human fetal lungs of either gender which were divided into five subgroups of various gestational ages ranging from 15 weeks to 40 weeks. The morphological details of the appearance of fissures of lungs and the presence of any variation were noted. Results: Out of 32 specimens of the right fetal lung, the horizontal fissure was absent in two fetuses. The oblique fissure was complete and present in all 32 specimens of the right lung. Among 32 specimens of the left lung, the absence of oblique fissure was observed in one lung. No accessory fissure and lobe was seen. Conclusion: In our study, there was no correlation between the appearance of fissure and gestational age as the absence of horizontal fissure in the right lung is found among two gestational age groups; one in 21–25 weeks and one in 36–40 weeks. Furthermore, the absence of oblique fissure in the left lung is seen in the age group of 36–40 weeks. However, it was found that oblique fissure in both lungs and horizontal fissure in the right lung appear as early as 15 weeks of gestation.

Incidence of furcate umbilical cord

Background : Placenta along with its umbilical cord is a vital organ for maintaining pregnancy and promoting normal foetal development. Foetal outcome can be adversely influenced by pathological changes in placenta and also by the variation in the site of attachment of umbilical cord. Aims and Objectives : To study the incidence of furcate umbilical cord insertion over placental chorionic plate in normal full term pregnancy. Materials and methods : This study was conducted in the department of Anatomy Government Medical College Haldwani. 100 freshly delivered placentae were collected from Dr. Sushila Tiwari Government hospital Haldwani. The sample was further categorized according to the parity of mother into two groups primipara (n=39) and multipara (n=61). Placentae included in the study were obtained from full term pregnancy without any complication like diabetes, hypertension etc. Results : The incidence of furcate umbilical cord in the present study was 2.6% in primipara group and 3.3% in multipara group. Conclusion : Variation in the site of insertion of umbilical cord are thought to result from process known as "Trophotropism" in which the chorionic frondosum or the early placenta migrates with advancing gestation to ensure better blood supply from more richly vascularised area. The overall incidence of furcate cord in this study is 3%. We have noticed one furcate cord (2.6%) in primipara group (n=39) and two furcate cords (3.3%) in multipara group (n=61). The neonates delivered were perfectly healthy without any evidence of congenital anomalies.

Histogenesis of human fetal spleen

Background and Aims: Spleen is the largest secondary lymphatic organ. It acts as a graveyard for RBCs, is essential for immune responses, performs lymphopoiesis in adults and haemopoiesis in fetuses. The present study was conducted to assess the histogenesis of spleen in human fetuses in view of existing literature. Material and Methods: The study was carried out on 34 formalin preserved human fetuses procured from Dr Sushila Tiwari Government Hospital, Haldwani with due clearance from ethical committee. The 6 |jm sections of the spleen were stained with Haematoxylin and Eosin and observed under light microscope. Results: At 14 tol5 weeks, spleen had extensive sinusoids filled with RBCs and few lymphocytes. At 16-18 weeks, trabecular arteries were noticed more towards centre along with extensive haemopoietic cells in the venous sinusoids. By 20th week lymphocytic aggregation had started around arterioles. By 24 weeks periarteriolar lymphatic sheath was clearly observed. At term (37-40 weeks), classical primary lymphoid follicle was present but germinal centers were not observed. Conclusion: During earlier differentiation, spleen symbolizes the function of haemopoietic activities and gradually during subsequent gestation; it establishes its identity as a principle lymphoid tissue.

CHRONOLOGY OF HISTOLOGICAL CHANGES IN HUMAN FOETAL URETER

Objective: Foetal ureter is developed by ureteric bud shows many developmental changes. Normal histology of the fetal ureter at various stages of development was studied to get insight into the morphology of fetal ureter. It is necessity for correlation with increase in gestational age and pathological changes for normal functioning of urinary system in fetal life. Method: Dissection of 87 normal human fetuses was done and histological findings of ureter were noted with respect to the age. The histology was studied by using H & E and Masson’s trichome stain. Important developmental stages were studied. Result: chronological fetal ureteral histology is different from adult Unlike in adult ureter, oval shaped lumen shows multiple epithelial layers with circularly arranged, At 12 weeks, the smooth muscle layer was in a single layer with an abundance of interspersed collagen fibers. Vascularization increases with age in lamina propria and division of muscular layer as inner longitudinal and outer circular layer occurs. Development of transitional epithelium increased in layers by 34 weeks in the lumen. Later on, the lumen changes to star shaped by multiple mucosal folds. At 36 weeks, the thickness of the wall increased significantly with a decrease in collagen. There was no trace of longitudinal muscle fibers even upto 33 weeks. Conclusion: Epithelial, muscular and connective tissue components displayed significant changes during intra uterine development The present study will be helpful in understanding the normal histological architecture of Foetal ureter and add to the existing knowledge regarding its development, its relation with gestational age and pathogenesis. Unilateral Duplication of Ureter

Duplication of ureter may be partial or complete. In partial duplication proximally separated two limbs join distally to form one ureter before entering the urinary bladder. Complete duplication where two ureters remain separated through out the course with separate openings in the urinary bladder is less common. Complete duplication of ureter occurs in 0.2% of live births. The prevalence of partial duplication found on urograms is 0.6%.

An unusual case of death due to Lightening

An electrical discharge between a negatively charged thunder cloud and a positively charged object on the earth causes lightening. A single flash which is of an enormous potential last about 1/1000 of seconds. The effect of lightening estimated about 100–1000 million volts with current of about 2000amperes. The effect of lightening on the human body depends on number of feature such as intensity of the current, the time it spend passing through the body and the pathway involved. Dry skin and dry clothes are bad conductors where as wet clothes and wet skins are good conductors.

Meckel-Gruber Syndrome with Multiple Congenital Anomalies- A Rare Lethal Case

Meckel-Gruber Syndrome (MGS), a rare autosomal recessive malformation syndrome with incidence of one per 13,250-140,000 live birth. It is a classical triad of occipital encephalocele, infantile polycystic kidneys and postaxial polydactyly with associated multiple congenital anomalies. It leads to the death of foetus in utero or shortly after birth. Most important diagnostic tools for MGS are antenatal ultrasonography and chromosomal study. A 3.14 kg dead female foetus was delivered from 19-year-old primigravida on the first visit to hospital with full term pregnancy. Sonography revealed no cardiac activity of the foetus in utero. Termination of pregnancy was done by the decision of obstetrician and consent of patient. Gross multiple anomalies along with microanatomical changes were noticed in foetus. Typical triad of polycystic kidney resulted to oligohydramnios, protrusion of meninges with brain tissue resulted to occipital encephalocele and limb anomalies might be hereditary or due to mutational changes. Histological changes were supportive of gross anatomical changes and ultrasound scan findings. Major changes as multiple cystic lesions in kidney, non functional alveoli in lungs and fibrous tissues with deranged hepatocytes in foetal liver were noticed. Concluding through all investigations and family history might confirm the diagnosis and help in further treatment or termination of pregnancy timely and directs for further genetic and chromosomal counselling.
