Increased CD14+HLA-DR−/low Myeloid-Derived Suppressor Cells Correlate With Disease Severity in Systemic Lupus Erythematosus Patients in an iNOS-Dependent Manner

Myeloid-derived suppressor cells (MDSCs) comprise of a population of cells, which suppress the innate and adaptive immune system via different mechanisms. MDSCs are accumulated under pathological conditions. The present study aimed to clarify the pathological role of MDSCs in systemic lupus erythematosus (SLE) patients. Consequently, the level of circulating M-MDSCs was significantly increased in newly diagnosed SLE patients as compared to healthy controls. An elevated level of M-MDSCs was positively correlated with the disease severity in SLE patients and an immunosuppressive role was exerted in an iNOS-dependent manner. The decrease in the number of M-MDSCs after therapy rendered them as an indicator for the efficacy of treatment. These results demonstrated that M-MDSCs participated in the pathological progress in SLE patients. Thus, MDSCs are attractive biomarkers and therapeutic targets for SLE patients.

CD34+ CD38- subpopulation without CD123 and CD44 is responsible for LSC and correlated with imbalance of immune cell subsets in AML

Acute myeloid leukemia (AML) is regarded as a stem cell disease. However, no one unique marker is expressed on leukemia stem cells (LSC) but not on leukemic blasts nor normal hematopoietic stem cells (HSC). CD34+ CD38- with or without CD123 or CD44 subpopulations are immunophenotypically defined as putative LSC fractions in AML. Nevertheless, markers that can be effectively and simply held responsible for the intrinsical heterogeneity of LSC is still unclear. In the present study, we examined the frequency of three different LSC subtypes (CD34+ CD38-, CD34+ CD38- CD123+ , CD34+ CD38- CD44+ ) in AML at diagnosis. We then validated their prognostic significance on the relevance of spectral features for diagnostic stratification, immune status, induction therapy response, treatment effect maintenance, and long-term survival. In our findings, high proportions of the above three different LSC subtypes were all significantly characterized with low complete remission (CR) rate, high relapse/refractory rate, poor overall survival (OS), frequent FLT3-ITD mutation, the high level of regulatory T cells (Treg) and monocytic myeloid-derived suppressor cells (M-MDSC). However, there was no significant statistical difference in all kinds of other clinical performance among the three different LSC groups. It was demonstrated that CD34+ CD38- subpopulation without CD123 and CD44 might be held responsible for LSC and correlated with an imbalance of immune cell subsets in AML.

Diagnostic value of flow cytometry in aggressive natural killer-cell leukemia: a case report and review of literatures

Objective To explore clinical characteristics and diagnosis of aggressive natural killercell leukemia (ANKL),and evaluate the value of flow cytometry (FCM) in diagnosing it.Methods A case of ANKL was reported and literatures were reviewed.Results The patient presented with persistent high fever,progressive pancytopenia and hepatosplenomegaly.The untypical cells could be seen by morphology.By FCM,NK cells consisted 83.3 ％ of total lymphocytes in bone marrow and immunophenotypes were CD34-,CD2+,CD7+,CD3-,CyCD3+,CD5-,CD16+,CD56+,CD30-,CD4-,CD8-,CD117-,CD11c,CD19-,CD45++,SSC+-++.T-cell receptor (TCR) and IgH gene rearrangement were negative and chromosome was normal.The patient was diagonised ANKL eventually.Conclusions ANKL is a quite rare disease with highly aggressive,poor prognosis and could be misdiagnosed easily.FCM combined with morphology is a convenient,handy,practicable and less invasive device for the diagnosis,and can be a preferred detection technique in some cases. IL-15 is an important pro-inflammatory factor.It plays an important role in natural immunity.Rheumatoid arthritis,psoriasis,systemic lupus erythematosus,and many other autoimmune diseases are closely related to the abnormal expression of IL-15.There is a complex regulatory mechanism in IL-15 gene expression.Recent studies found a variety of factors with different mechanisms involved in IL-15 gene expression regulation in various stages,especially in the level of transcription by monocytes/macrophages-stimulating factor and in the level of post-transcription by two kinds of specific signal peptides.The recent discovery of IL-15 negative feedback regulation by exogenous IL-15 and SSP-IL-15 Was also discussed. Immune dysregulation in primary immune thrombocytopenia patients

Objectives: To explore the immunological abnormalities in patients with primary immune thrombocytopenia (ITP), and analyze its relationship with treatment.Methods: Proportion of different immune cell subsets were detected in the peripheral blood of 124 ITP patients at different time points and 45 normal controls by flow cytometry. The treatments included glucocorticoids, intravenous IgG as first-line treatment and second-line drugs.Results: Elevated CD4/CD8 ratio and decreased the proportion of NK and CD4 + CD25 + CD127low regulatory T cells (Tregs) were found in pre-treated ITP patients than healthy controls. The newly diagnosed group had a significantly higher CD4/CD8 ratio than the relapsed group, but no differences in the proportion of B cells, NK cells and Tregs. No relationships were found between the curative effect and the pre-treated cell subsets within both the effective and ineffective groups. Furthermore, compared with the ineffective group, the effective group had higher Tregs and lower CD4/CD8 ratio post-treatment, but no significant differences in NK and B cells.Conclusion: ITP patients presented with a high CD4/CD8 ratio and low levels of Tregs and NK cells, suggesting that immune deregulation was involved in the pathogenesis of ITP. The pre-treated immune status of ITP patients may not be related to the curative effect. Tregs significantly increased in the effective group post-treatment, highlighting that the mechanism of restoring Tregs may be involved in the treatment of ITP. However, whether or not the targeted regulation of Tregs is an effective treatment for ITP still requires further studies.

SRRM2 is a target for the immunotherapy of AML with rationally designed CAR-T cells

Abstract Background: Acute myeloid leukemia (AML) is a highly aggressive hematological malignancy that necessitates innovative treatment strategies to improve patient outcomes. The chimeric antigen receptor T cell immunotherapy (CAR-T) has emerged as a groundbreaking approach in immunotherapy; however, the lack of an optimal target antigen remains a significant impediment for CAR-T in its application to AML treatment. The nuclear speckle marker, serine/arginine repetitive matrix 2 (SRRM2), encompasses multiple disordered domains and its dysfunction is implicated in various human diseases. Methods: We tested the expression of SRRM2 antigen on peripheral blood cells and bone marrow cells AML patients. The clinical data of patients collected for studying the correlation between SRRM2 expression and clinical parameters. Then, a chimeric antigen receptor (CAR) containing a SRRM2-specific single-chain variable fragment, in combination with CD28, 4-1BB costimulatory domains, and CD3-ζ signaling domain was developed. LDH release assay was used to evaluate the in vitro cytotoxicity of the SRRM2 CAR-T cells. An AML mouse model of SKM-1 was established to evaluate the in vivo anti-leukemic activity of the SRRM2 CAR-T cells Results: In this study, we have observed the presence of SRRM2 expression on the cellular membrane. The membrane-expressed SRRM2 is elevated in AML patients and AML cell lines, particularly in individuals and cell lines harboring fms-like tyrosine kinase 3 (FLT3) gene mutations, making it an optimal target for CAR-T therapy. Then we designed and constructed a CAR, which targets SRRM2, involved the incorporation of SRRM2-specific single-chain variable fragments (ScFv), CD28 and 4-1BB costimulatory domains, as well as CD3-ζ signaling domains. The functionality of SRRM2 CAR-T cells was further investigated, revealing their specific cytotoxicity against SRRM2 + AML cell lines and enhanced potency against FLT3-mutant cells. In vivo SRRM2 CAR-T therapy of AML mice demonstrated the remarkable safety and efficacy profile. Conclusion: These results support that SRRM2 CAR-T represents a promising immunotherapy for the treatment of AML.
