A Base‐ and Metal‐free Protocol for the Synthesis of 2‐Aryl/heteroaryl Thiazolines

Abstract A simple and efficient base‐ and metal‐ free protocol for the synthesis of 2‐aryl/heteroarylthiazoline from aryl/heteroaryl nitriles and cysteamine hydrochloride is reported. This method is applicable to a series of aryl/heteroaryl nitriles containing different substituents such as halides, amines, substituted azoles, etc. and furnishes the corresponding thiazolines in upto 99% yields. A selective synthesis of mono‐ or bis‐thiazoline in more than 90% yield was also observed by modifying the duration of the reaction. This novel methodology has been utilized for the synthesis of some important 2‐aryl/heteroarylthiazoline scaffolds which form a part of many biologically active molecules. A probable reaction mechanism involving the formation of highly activated arylimino chloride is also given.

Fine‐needle aspiration of cervical lymph nodes yields adequate materials for accurate HPV testing in metastatic head and neck squamous cell carcinomas

Background High‐risk Human papillomavirus (HR‐HPV)‐associated head and neck squamous cell carcinoma (HNSCC) is a distinct epidemiologic and pathologic disease. The data of HR‐HPV testing on fine‐needle aspiration (FNA) materials of cervical lymph nodes in patients with metastatic HNSCC are limited. Methods We retrospectively reviewed metastatic HNSCC cases with cervical lymph node FNAs which were tested for p16 immunohistochemistry (IHC) and HR‐HPV DNA in situ hybridization (ISH). Results Of the 87 cases studied, the overall adequacy rate of cell block material was 86% for HR‐HPV ISH, 93% for p16 IHC, 84% for both and 95% for either one. Pathologist‐performed FNAs showed significantly higher percentage of satisfactory specimens for p16 and HR‐HPV tests than clinician‐performed FNAs. Of the 73 cases adequate for both p16 IHC and HR‐HPV ISH, the concordant rate between p16 and HR‐HPV results was 78%. P16 positivity had sensitivity of 90% and specificity of 75% to predict primary oropharyngeal HNSCC, while HR‐HPV test showed a similar high sensitivity (91%) but a better specificity (94%). Conclusion FNAs of cervical lymph nodes yield adequate materials for reliable HR‐HPV testing in metastatic HNSCCs and should be incorporated into routine pathologic evaluation for these patients. The high concordance between p16 IHC and HR‐HPV ISH suggests p16 IHC may serve as a surrogate for HR‐HPV ISH in metastatic HNSCC cases with oropharyngeal origin and is helpful to pinpoint primary tumor site. Clinical Evaluation of Overall Efficacy and Pain Perception of Ultrasonic Oscillating Tips and Conventional High-speed Burs for Removal of Dental Caries in Children in Age-group of 6–8 Years

To clinically evaluate the overall efficacy and pain perception of ultrasonic oscillating tips and conventional high-speed burs to remove enamel and dentinal caries in children of ages 6-8 years.Bilateral deciduous canines with caries involving enamel or enamel and dentin were excavated by using a high-speed airrotor on one tooth and ultrasonic oscillating tips on another tooth in the same appointment. The overall response of the patient, both ultrasonic oscillating tips and airrotor along was recorded by Wong-Baker's Facial Pain Rating Scale (WBFPS) along with time taken to make each cavity and noise level generated by high-speed and ultrasonic system.Evaluation of WBFPS after every method of caries excavation showed that 84% of subjects in the ultrasonic group reported score of 0 as compared to only 24% of subjects in the airrotor group. Score 2 was reported by 16% of the subjects in the ultrasonic group and 32% of the subjects in the airrotor group. Score 4 was reported by 44% of the subjects in the airrotor group. Ultrasonic groups reported more time to remove caries as compared to the airrotor group. The ultrasonic group reported less noise as compared to the airrotor group and the difference between the two was significant.Patients preferred the ultrasonic method for caries excavation because of less pain and noise. The use of ultrasonic oscillating tips is as effective in caries excavation, less painful, and more time-consuming than the conventional airrotor.Ultrasonic oscillating tips can be successfully used in pediatric dentistry to aid patient cooperation and reduce pain during caries excavation.Kenjale MA, Shah P, Desai S, et al. The key factor in success of dental implants is its ability to integrate with the surrounding bone, or in short, osseointegration. The events leading tosuccess or failure of osseointegration of a dental implant takes place mostly at the tissue- implant surface. It would be no doubt to say, that the implant surface offers the greatest potential to alter the process of osseointegration.Although the main purpose of surface modification of implants is to achieve better osseointegration, a shortened period of healing is desirable for both the clinician and the patient.

A rare variant innervation of adductor pollicis muscle from median nerve in an adult cadaver in middle east: a case report

Few sporadic reports have been found in past and some of them described this as an etiological factor for incomplete nerve decompression or erroneous results during electrophysiological studies.Here we describe a rare case of adductor pollicis muscle in hand which was supplied from branch arising from lateral ramus of median nerve.In past, such variation in innervation of adductor pollicis brevis has never been reported in this region.Presence may be attributed to rerouting of nerve fibers at various levels.Particularly in light of the increasing number of nerve conduction studies, knowledge of normal and variant anatomic features has become more important than in the past..Knowledge about variant innervation pattern would thus aid in correct explanation of results during motor testing of hypothenar and thenar muscles and while interpreting variant symptomatology in carpal tunnel syndrome and muscle contractures.Prior academic knowledge in nerve supply of muscles are extremely important and awareness would thus help to prevent iatrogenic injuries and better prognosis in cases of nerve decompression procedures, muscle flap transplant and various graft surgeries.
