In vitro potency of combinatorial antifungal drugs towards Candida isolates from patients

Abstract Candidiasis is a fungal infection (mycosis) caused by an opportunistic yeast with only 8-10 pathogenic species. In spite of the multiple classes of antifungal drugs available to combat these infections, the treatment is hampered by drug toxicity, tolerability, emergence of drug resistant isolates and many more. Combination therapy has been suggested as a possible approach to improve these treatment outcomes. This study is, therefore, aimed at the analysis of the sensitivity of Candida isolates obtained from patients towards different classes of drugs affecting multiple pathways in the pathogen life cycle. Four different individual drugs with different mechanisms of action and six different combinations of these drugs at three different proportions (1:1, 1:3, and 3:1) were used for the study. All the drug sensitivity assays were performed by the agar well diffusion method. The results were statistically analyzed using Prism software. Results have shown synergistic effects of drug combinations on the sensitivity of the isolates without much variation in the stoichiometric ratios, thereby providing an alternative to monotherapy to combat the emergence of drug resistant isolates and drying existing drug pipelines.

Differential Effects of Cholesterol and its Immediate Biosynthetic Precursors on Membrane Organization

Cholesterol is the most representative sterol present in vertebrate membranes and is the end product of the long and multistep sterol biosynthetic pathway. 7-Dehydrocholesterol (7-DHC) and desmosterol are the immediate biosynthetic precursors of cholesterol in the Kandutsch-Russell and Bloch pathway. In this article, we have monitored the effect of cholesterol and its two immediate biosynthetic precursors on biophysical and dynamic properties of fluid and gel phase membranes. Toward this goal, we have used fluorescent membrane probes, DPH and TMA-DPH, and the hydrophobic probe, pyrene. Our results using these probes show that although both 7-DHC and desmosterol differ with cholesterol in one double bond, they exhibit differential effects on membrane organization and dynamics. Importantly, we show that the effect of cholesterol and desmosterol on membrane organization and dynamics is similar in most cases, while 7-DHC has a considerably different effect. This demonstrates that the position of the double bond in sterols is an important determinant in maintaining membrane order and dynamics. These results assume relevance since the accumulation of cholesterol precursors have been reported to result in severe pathological conditions.

The mechanism of chloroquine (CQ) resistance in Plasmodium falciparum is not clearly understood. However, CQ resistance has been shown to be associated with point mutations in Pfcrt and Pfmdr1. These genes encode for digestive vacuole transmembrane proteins Pfcrt and Pgh1, respectively. The present study was carried out to analyze the association of Pfcrt-K76T and Pfmdr1-N86Y mutations with CQ resistance in Northeast Indian P. falciparum isolates. 115 P. falciparum isolates were subjected to in vitro CQ sensitivity testing and PCR-RFLP analysis for the Pfmdr1-N86Y and Pfcrt-K76T mutations. 100 isolates of P. falciparum were found to be resistant to CQ by the in vitro susceptibility test (geometric mean EC50 2.21 µM/L blood) while 15 were found to be CQ sensitive (geometric mean EC50 0.32 µM/L blood). All the CQ resistant isolates showed the presence of Pfmdr1 and Pfcrt mutations. CQ sensitive isolates were negative for these mutations. Strong linkage disequilibrium was observed between the alleles at these two loci (Pfmdr1-N86Y and Pfcrt-K76T). The results indicate that Pfmdr1-N86Y and Pfcrt-K76T mutations can be used as molecular markers to identify CQ resistance in P. falciparum. The result necessitates the evaluation of CQ in vivo therapeutic efficacy in endemic areas for more effective malaria control strategies.

Comparison of Broth Dilution and Disc Diffusion Method for the Antifungal Susceptibility Testing of Aspergillus flavus

With the discovery of newer antifungal agents, emergence of resistance among Aspergillus, is a major problem.In the present study, 25 different clinical isolates of Aspergillus flavus were screened for their susceptibility against five common antifungal agents by broth dilution and disc diffusion method with the aim to evaluate both the methods for their suitability to adapt in clinical laboratory.All the isolates showed susceptibility to five antifungals by broth dilution after 24 hrs however all isolates showed resistance for 5FC, fluconazole and ketoconazole after 48 hrs.By disc diffusion method, all isolates were sensitive for all antifungals except fluconazole which showed resistance even after 24 hrs.Amphotericin B and itraconazole were in good agreement between both the methods where 100% isolates were sensitive at both time period by broth dilution and 84% to 100% by disc diffusion method.Both the methods are in good agreement and since disc diffusion method is easy to perform, cheap, rapid and same can be adapted easily for the routine susceptibility testing of clinical fungal isolates in medical microbiology laboratory.
