Two-drape closed pocket technique: minimizing aerosolization in mastoid exploration during COVID-19 pandemic

Mastoidectomy is associated with extensive bone-drilling which makes it a major aerosol generating procedure. Considering the ongoing COVID-19 global pandemic, it is essential to devise methods to minimize aerosolization and hence ensure safety of the healthcare workers during the operative procedure. Two disposable surgical drapes are used to create a closed pocket prior to commencement of mastoid bone-drilling. This limits aerosolization of bone-dust in the external operating theatre environment. Two-drape closed pocket technique is an easy, cost-effective and safe method to limit aerosolization of tissue particles during mastoidectomy.

Type 1 tympanoplasty: does the status of contralateral ear affect the outcome?

Background: The objective of the study was to investigate the effect of contralateral ear status on the success rate (anatomical closure) of type 1 tympanoplasty.Methods: The study is a prospective observational study comprising of 60 patients diagnosed with chronic otitis media, who underwent type 1 tympanoplasty during 2016-2018 in a tertiary care hospital. Results: In our study, contralateral ear was normal in 40 (66.7%) cases & diseased in 20 (33.3%) cases. Success rate of type 1 tympanoplasty in patients with normal contralateral ear was 90% (n=36) but success rate was only 60% (n=12) in diseased contralateral ear. This was statistically significant (p=0.006).Conclusions: Our study revealed that the status of the opposite ear is an individual prognostic factor for type 1 tympanoplasty. In other words, graft-healing rates are poorer in individuals whose opposite ears are atelectatic or perforated because of chronic otitis media.

Ninety percent cancers in the head and neck area are squamous cell cancers (HNSCC) from mucosal epithelium. But anatomical sub sites and morphological sub varieties have different biology, patterns of spread, and treatment requirements. Temporal trend of incident of HNSCC in India is declining; but tongue and oropharynx cancers show a rise in younger age groups with the types that have been reported to be HPV related in western countries, which raises the issue of vaccines and screening as in cervical cancers. Use of MRI, MR spectroscopy, 3D CT scan with reconstruction, and PET-CT in clinics allow more accurate staging and post therapy monitoring. Molecular biology tools now can divide the conventional HNSCC in two main groups: 1) elderly male, user of tobacco and alcohol, keratinizing SCC, TP53 mutated and/or p53-positive, HPV16-negative, with an aggressive course; and 2) younger men without known risk factors, non-keratinizing SCC, HPV16-positive, p16-positive, and p53-negative, with improved prognosis. While surgical treatment has expanded toward two opposite but complementary directions of minimally invasive to ultra radical excision and reconstruction, radiotherapy has moved to a more biological approach by altering dosage schedule, being more conformal and using chemotherapy concurrently. Neoadjuvant or Induction chemotherapy has been introduced before definitive treatment with surgery or radiation alone or concurrent chemoradiotherapy. Targeted therapy with cetuximab in combination with chemotherapy and radiotherapy has improved results in advanced or recurrent disease. Many other anti growth factor agents like TKIs and anti VEGF monoclonal antibody, bevacizumab are showing promise.

Endoscopic ossiculoplasty: Is there any edge over the microscopic technique?

Objectives/Hypothesis To compare endoscopic ossiculoplasty with conventional microscopic technique in terms of postoperative hearing outcomes and complications. Study Design Randomized controlled trial. Methods One hundred eighteen patients diagnosed with ossicular chain discontinuity were randomly assigned into two groups, one undergoing endoscopic ossiculoplasty and the other undergoing ossiculoplasty by the microscopic technique, with the operating surgeon being same for both groups. The two groups were compared in terms of operative time, postoperative air‐bone gap, mean air‐bone gap closure, and incidence of complications. Teflon prostheses (partial ossicular chain replacement prosthesis [PORP] and total ossicular chain replacement prosthesis [TORP]) were used for reconstruction in all cases. Results Endoscopic ossiculoplasty with PORP rendered a statistically significant mean postoperative air‐bone gap and air‐bone gap closure at 1 month when compared to that of microscopic PORP ossiculoplasty. However, there was no significant difference between the two techniques in terms of mean postoperative air‐bone gap and air‐bone gap closure at 3 and 6 months. In the TORP ossiculoplasty cases, there was no significant difference in mean postoperative air‐bone gap and air‐bone gap closure at 1, 3, and 6 months. In terms of operative time and incidence of complications, no statistical significance was found between the two groups. Conclusions Endoscopic ossiculoplasty appears to provide superior visualization and better early audiological outcome (in PORP ossiculoplasty cases) when compared to microscopic technique. CHARACTERISTICS AND SPREAD PATTERN OF LARYNGEAL CARCINOMA : A TERTIARY CARE STUDY

INTRODUCTION Laryngeal cancer is the eighteenth most common cancer in the UK. It has strong socioeconomic association, wide geographical variations. This study is highly relevant in India where factors like poor socio-economic conditions, oral consumption of tobacco in its various forms, alcohol, smoking habits in form of beedi and cigarette, lack of awareness about cancer, negligence towards the symptom of voice change and primary treatment from quacks , are highly prevalent. MATERIALS AND METHODOLOGY This prospective cohort study was carried out at Tertiary care Hospital over a period of 1 year 4 months( April 2018 to July 2019). Patients attending at our OPD with symptoms of horseness, dysphagia, stridor and other symptoms of Laryngeal Carcinoma were subjected to detailed clinical examination including FOL to confirm the presence of any growth. All patients with growth or vocal cord irregularity or any suspicious lesions underwent biopsy. Socio-demographic, risk factors , characteristics of primary tumor- endoscopic, radiological and histopathological and spread pattern and node status analysed. RESULTS Major bulk of patients belonged to 55-74 years (combined). The mean age of diagnosis is 66.06 years.Males were affected more than females (7.57:1).Majority of patients were farmers (57%) and majority 45(75%) belonged to rural areas. Most patients belonged to low socioeconomic class 30 (50%).Smoke tobacco and smoke tobacco plus alcohol were the major risk factors for laryngeal carcinoma.Majority of patients presented with dysphagia (75%), followed by foreign body sensation (72%), hoarseness (67%), Neck swelling (50%). Cartilage invasion was present among 25% of cases , Both pre-epiglottic and paraglottic space involvement was around 18% and exolaryngeal spread present in 8% of cases.Among them 100% cases delayed symptom recognition was present. 50% cases was attributed due to socio-demographic pattern, 33% due to pshycosocial and behavioural (anxiety) and 13% due to delay in practitioner referral.Among Proliferative and Ulceroproliferative N+ is more than N0.Both moderately differentiated and poorly differentiated had more percentage of N+.In Supraglottic tumor Pre-epiglottic space, Paraglottic Space and cartilage invasion was present in equal proportions. In glottic tumor cartilage invasion and exolaryngeal spread was more common. CONCLUSION Supraglottic tumor being common have tendency for lymphatic spread. It mainly presents with dysphagia and hoarseness later. Majority of patients were diagnosed in stage III and stage IV. This is not desirable and causes for delayed diagnosis must be addressed publicly. Early stage tumor have excellent prognosis with advent of radiotherapy and surgical morbidities can be avoided. The HPE reveals high grade of differentiation is associated with low node status. CT features serve as an excellent tool in identifying spread and node involvement and also management protocol.

Ossiculoplasty using Autologous Reshaped Incus and Teflon PORP: A Comparative Study

Introduction Autologous reshaped Incus and Teflon partial ossicular replacement prosthesis (PORP) are commonly used for ossicular chain reconstruction. The present study attempts to assess the post-operative outcome with these two prostheses. Materials and Methods A Randomized prospective study was conducted in Tertiary referral care hospital to determine which material, among autologous reshaped Incus and Teflon partial ossicular replacement prosthesis (PORP) gives better postoperative hearing result in Ossiculoplasty. Patients presenting at outpatients’ department with the clinical diagnosis of chronic otitis media with perforation or retraction. The study population consisted of a total of 50 patients. Ossiculoplasty with reshaped Incus or PORP was performed after Canal Wall Up mastoidectomy. Hearing results were measured by Air-Bone gap in PTA after 6 months of surgery. Results Selecting the criteria <20 dB ABG as success when Stapes superstructure is present, Incus has 64.29% success rate, whereas PORP has 31.81% success. Extrusion rate of different prosthesis shows PORP has 18.18% extrusion whereas autologous Incus has lower (7.14%) extrusion rate. Conclusion Among these two ossiculoplasty materials, autologous Incus gives better postoperative hearing gain and lower extrusion rate.

Evaluating the Feasibility of Mobile Screening Units for Early Detection of Cancer in the Community

Abstract Background: Mobile screening units (MSUs) offer an effective approach to increasing access to cancer screening and reducing geographical and financial barriers. This study reports the outcomes of MSUs in cervical, breast, and oral cancer screening. Methodology: The MSU was integrated into a bus with three compartments, with designated areas for registration, cervical cancer screening, oral visual examination (OVE), and mammography. Between May 2023 and August 2024, 178 community camps were held in Kolkata and nearby districts. A total of 6205 women underwent cervical, breast, and oral cancer screening. Women aged 30–60 years were screened for cervical cancer by high-risk human papillomavirus (hrHPV) DNA test while breast cancer screening was conducted using clinical breast examination. Any abnormal findings on examination were referred for mammography. Results: HrHPV positivity was detected in 323 (5.3%) women by Cobas assay. Following this, 218 women were referred for colposcopy and treatment. Following clinical breast examination, 316 women were referred for mammography. In addition, 28 women with suspicious breast lesions were referred to the Chittaranjan National Cancer Institute for further management. Abnormal OVE was diagnosed in 18 women who were advised to quit tobacco. Conclusion: MSUs serve as a valuable tool for expanding access to cancer screening in resource-limited settings and reduce the global burden of preventable cancers as well as meet cancer screening targets.

A comparative study of common ear morbidity pattern among the primary school children of an urban slum of Kolkata and rural area of Hooghly.

A cross-sectional, clinical and epidemiological study was undertaken among 627 primary school children (rural 145, urban 482) to compare the common ear morbidity pattern between an urban slum of kolkata and a rural area of Hooghly. Middle ear pathology was found to be present in 20% and 12.6% among rural and urban students respectively. Cerumen in the external auditory canal was the commonest clinical finding in both the areas and was found to be present in 35.86% of rural and 30.70% of urban population respectively. Smoke nuisance, bathing in open ponds and overcrowding were some of the predisposing factors causing ear diseases, like chronic suppurative otitis media and serous otitis media.
