Unraveling the Role of Repurposed Drugs in the Treatment of Acne: Success so Far and the Road Ahead

Acne is a skin disease that impacts 9.4% of the world's population. Available treatments for managing acne include retinoid-like drugs, antibiotics, corticosteroids, photo, and radiotherapy. Howevere, the aforementioned treatments have certain limitations such as possibility of developing skin cancer from tetracycline, doxycycline, and corticosteroids, microbial resistance to antibiotics, and deadly side effects, and so forth. Repurposing of existing therapeutics having excellent safety profile can be promising way to treat acne efficiently. The repurposed drugs and phytoceuticals from diverse classes have demonstrated promising effects in treating acne. These repurposed drugs have displayed antiacne effectiveness by targeting single or multiple signaling pathways. Various repurposed therapeutics undergoing clinical trials at different phases demonstrated their safety and efficacy in treating acne. Despite being a very good, safe, and less time-consuming strategy, drug repurposing (DR) faces multiple challenges such as lack of regulatory guidelines, preservation of intellectual property, and clinical validation of claimed therapeutic indication. DR appears to be a viable approach and is likely to offer effective treatment at a reasonable cost in alleviating acne.

Response assessment by positron emission tomography‐computed tomography as compared with contrast‐enhanced computed tomography in childhood Hodgkin lymphoma can reduce the need for radiotherapy in low‐ and middle‐income countries

The InPOG-HL-15-01, a multicentric prospective study, used a risk-stratified and response-based approach with doxorubicin, bleomycin, vinblastine, and dacarbazine (ABVD) backbone to treat children and adolescents with newly diagnosed Hodgkin lymphoma (HL) and reduce the use of radiation therapy (RT). Children/adolescents with bulky disease or inadequate response at early response assessment (ERA) after two cycles of chemotherapy were assigned to receive RT. For ERA, positron emission tomography computed tomography (PET-CT) was recommended but not mandatory in view of limited access. This study aimed to compare the impact of using contrast-enhanced computed tomography (CECT) and PET-CT on treatment decisions and outcomes.396 patients were enrolled and 382 had an ERA at the assigned time point. Satisfactory response was defined as Deauville score 3 or less for patients undergoing PET-CT and complete response (CR)/very good partial response (VGPR) for patients undergoing CECT. Outcomes of interest incorporate 5 year event-free survival (EFS), EFS including abandonment (EFSa), and overall survival (OS).At ERA, satisfactory response was documented in 277 out of 382 (72.5%) participants and this was significantly higher in PET-CT (151 out of 186, 81.2%) as compared with CECT-based assessments (126 out of 196, 64.3%) respectively (p value < .001). Amongst the 203 patients with nonbulky disease (wherein the indication for RT was entirely dependent on ERA), 96 out of 114 (84.2%) and 61 out of 89 (68.5%) patients achieved a satisfactory response according to the PET-CT and CECT (p value = .008) respectively and hence a lesser proportion of patients in the PET-CT arm received RT. Despite a lower usage of RT the 5 year OS of both groups, ERA based on CECT (91.8%) versus PET-CT (94.1%) was comparable (p value = .391) and so was the 5 year EFS (86.7 vs. 85.5%, p value = .724).Use of PET-CT as the modality for ERA is more likely to indicate a satisfactory response as compared with CECT and thereby decreases the need for RT in response-based treatment algorithm for HL-afflicted children. The reduction in the application of RT did not impact the overall outcome and plausibly would lower the risk of delayed toxic effects.

Rauwolfia serpentina is reported in an Ayurvedic medicinal system for centuries, for the treatment of various ailments such as snakebites, insomnia, hypertension, and insanity. Scientific evaluation of these documents can be valuable for finding new potential use in neurological disorders. The work presents the brief overview of R. serpentina including a description of the plant, its active chemical constituents and pharmacological properties with the major emphasis on cardiovascular and central nervous system disorders. This review compiles information available in the scientific literature from databases such as Science Direct, PubMed, India bioscience.org, Herbs - Medicinal plant usage and Identification Database, Database on medicinal plants used in Ayurveda and Siddha, Missouri Botanical Garden, National Medicinal Plants Board, and the International Plant Names Index. Information gathered from the literature has shown that the alkaloids are the major constituents of the plant imparting various pharmacological properties. Reserpine, the Indole alkaloid, is the most active compound of R. serpentine. The plant is known to possess antidiarrheal, antimicrobial activity apart from using it for the treatment of circulatory disorders, rheumatism, hypertension, insanity, epilepsy, and leaves are used in the removal of opacities of the cornea. Research shows that R. serpentine is a potential source of compounds pertaining medicinal applications. It provides an interesting subject in the search for new drugs of natural origin.

This review summarizes the most common types of psychosomatic disorders including various types of depression based on their evolution, causes, symptoms and severity. The cellular and molecular mechanism behind the same in the areas of the brain, which are most susceptible to damage, both structurally and functionally (amygdala, the prefrontal cortex and the hippocampus) were keenly looked in to. Further, the therapeutic intervention through antidepressants was also been explored along with its three major categories in which they have been divided, their types, properties and mechanism of actions, especially towards the regulation of neurotransmitters in the brain has been highlighted. The behaviour of neurotransmitters in the presence of particular antidepressants is of grave importance in order to design an effective antidepressant drug, which could beat currently ineffective ones in the market and their potential forms have been studied and described. This review also provides an insight about the different approaches used for diagnosis using different types of biomarkers.

Nano Adsorbents in Wastewater Treatment: A New Paradigm in Wastewater Management

Increasing water pollution has urged the need to elucidate more potent techniques for removing contaminants from wastewater 1 and groundwater. Numerous techniques or processes have been explored and designed for water purification. Amongst all other water/wastewater remediation techniques, nanotechnology has gained wider attention in water remediation via numerous mechanisms, such as the adsorption of heavy metals and the removal of pathogens. Nanoadsorbent materials have emerged as a promising solution for solving this crucial environmental issue. Their unique chemical and physical properties, such as higher ranking, quality, and status, promote their application compared to traditional adsorbents. Recent research studies have reported their promising potential in water treatment, including polymeric, carbon, tubezeolites, and metal nanosorbents. Hence, this review mainly aims to provide summarized data presenting all the beneficial aspects of nanosorbents in wastewater/water remediation.

Towards More Accurate Prediction of Human Empathy and Emotion in Text and Multi-turn Conversations by Combining Advanced NLP, Transformers-based Networks, and Linguistic Methodologies

Based on the WASSA 2022 Shared Task on Empathy Detection and Emotion Classification, we predict the level of empathic concern and personal distress displayed in essays. For the first stage of this project we implemented a Feed-Forward Neural Network using sentence-level embeddings as features. We experimented with four different embedding models for generating the inputs to the neural network. The subsequent stage builds upon the previous work and we have implemented three types of revisions. The first revision focuses on the enhancements to the model architecture and the training approach. The second revision focuses on handling class imbalance using stratified data sampling. The third revision focuses on leveraging lexical resources, where we apply four different resources to enrich the features associated with the dataset. During the final stage of this project, we have created the final end-to-end system for the primary task using an ensemble of models to revise primary task performance. Additionally, as part of the final stage, these approaches have been adapted to the WASSA 2023 Shared Task on Empathy Emotion and Personality Detection in Interactions, in which the empathic concern, emotion polarity, and emotion intensity in dyadic text conversations are predicted.

Aromatherapy or Essential oil therapy is attributed towards the field of conventional, alternative or complementary therapies which requires essential oils and other aromatic plant compounds. Essential oils (EOs) belong to the class of highly volatile compounds which, due to their higher concentrated natural constituents acquire unique properties and hence, are being used for its wide range of therapeutic health benefits. Their benefits widely range from being a potential antimicrobial, antidepressant, antiviral and antiseptic agents to the elevator of many CNS related behaviours like - anxiety, stress, wisdom, memory, attention, relaxation, sedation, and sleep. Furthermore, the effects on mood, pain, and perception too are being influenced by the use of essential oils. It also helps in the treatment of epilepsy, stress, dementia by giving calming and soothing effects to the nerve cells and is also effective against dementia and on the treatment of Alzheimer’s disease.

In-Silico Validation and Fabrication of Matrix Diffusion-Based Polymeric Transdermal Patches for Repurposing Gabapentin Hydrochloride in Neuropathic Pain

Background: Gabapentin (GBP) is an FDA-approved drug for the treatment of partial and secondary generalized seizures, apart from being used for diabetic neuropathic pain. GBP displays a highly intricate mechanism of action and its inhibitory response in elevated antagonism of NMDA (N-methyl-D-aspartate receptor) receptor and thus, can be repurposed for controlling neuropathic pain. Objective: Therefore, in the present study, we have selected hBCATc (humanPyridoxal 5’-phosphate- dependent branched-chain aminotransferase cytosolic) gene that is highly expressed in silico validation through neuropathic stressed conditions. Thereafter, have analysed the GBP as its competitive inhibitor by in silico validation through homology modelling, molecular docking, also predicting its structural alerts and pharmacokinetic suitability through ADMET. However, and GBP was found to be a potential drug in controlling neuropathic pain, still, it has certain critical and pharmacokinetic limitations; therefore, the need for its targeted delivery was required, and the same was attained by designing a GBP loaded transdermal patch (GBP-TDP). Methods: A suitable and equally efficacious GBP - TDP was developed by a solvent evaporation method using PVP and HPMC in the ratio of 2:1 as a polymer base for reservoir type of TDP. Also, PEG 400 was used as a plasticizer, and PVA (4%) was taken for backing membrane preparation, and then the optimized GBP-TDP was subjected for physical characterization, optimization and ex vivo release kinetics. Results: The results showed desired specifications with uneven and flaky surface appearance giving an avenue for controlled release of the drugs with 92.34 ± 1.43% of drug release in 10 hours, further suggesting that GBP-TDP can be used as an effective tool against diabetic neuropathy pain. Conclusion: In this study, we have repurposed Gabapentin to treat diabetic neuropathy and validated the same by conducting a detailed in silico evaluation starting from homology Modelling of the target protein hBCATc, cross verified by the Ramachandran plot analysis with the most favoured region of 92.1% (encompassing 303 residues out of 386).
