On rapid ascending to high-altitude particularly very high-altitude or extreme high-altitude, there is a risk of developing high-altitude illness and most people may experience acute mountain sickness which may further lead to potentially life-threatening pathologies like high-altitude pulmonary edema, high-altitude cerebral edema, high-altitude-induced thrombosis etc. if not treated on time. Hypercoagulability state associated with high-altitude which lead to the formation of a clot in the blood vessels, a condition called deep vein thrombosis, which may further complicate and lead to pulmonary embolism. Lack of epidemiological data poses a constraint in evaluating the actual incident rate of thromboembolic disorders at high-altitude. In the present scenario, the most commonly used diagnostic marker for thrombosis is the D-dimer test which has low specificity. Various anticoagulants are also available for anticoagulation therapy but they have their own limitation. Under this review, worldwide reported incidents and management strategies related to thrombotic complications are consolidated and presented. It also summarizes diagnostic and anticoagulation therapy regimes against thrombosis existing at present. Accurate diagnosis and therapeutics are a thrust area of further exploration and there is an urgent need to develop quick and advanced methods to reduce the mortality associated with this disorder especially with respect to high-altitude.

COVID-19 and Ischemic Stroke: Advancements and Challenges

Ischemic stroke has been illustrated to be an intermittent, life-endangering, complexity of COVID-19, affecting approximately 1–3% of admitted patients, and up to 6% of those in the intensive care units. Neuro-radiological abnormalities demonstrate the involvement of large vessels, multiple vascular territory infarction, small vessels, and vertebrobasilar territory occlusion. Studies speculate that cytokine storm-activated hypercoagulable condition, endotheliopathy, cardiac embolism, arterial dissection, and vasculitis-like mechanism may lead to its occurrence. Although the pathogenesis of stroke following a COVID-19 infection is multifarious and incompletely understood, however current evidence indicates crucial involvement of collective effects of virus-associated factors, medical condition of the host, and virus-host interactions. Amongst the hospital-admitted patients with COVID-19 infection, both prophylactic, as well as therapeutic anticoagulants, are capable of improvising hospital mortality rates and prognosis. In this chapter, we summarize the current knowledge on epidemiology, clinical manifestations, and pathophysiology of ischemic stroke in patients with COVID-19. We also aim to discuss the recent advancements and challenges for its diagnosis and rational management.

High Density Lipoprotein Dysfunctionality in Venous Thromboembolism

Venous thromboembolism (VTE) is complex multi‐factorial cardiovascular disease that encompasses deep vein thrombosis (DVT) and pulmonary embolism (PE) and has a high incidence in several populations across the world with unknown pathophysiological mechanism driven by various environmental, acquired and inherited risk factors. DVT, a condition where a blood clot forms inside deeply situated veins in the body (usually in the lower limbs) is very common among Indian population. Lipoproteins modulate the expression and function of coagulation/fibrinolytic factors in patients of various cardiovascular diseases. HDL associated proteins have been proven to show antioxidant, anti‐inflammatory and anti‐thrombotic properties. Thus, the present study aims to examine the functionality of HDL in VTE using an animal model and also validating the same in DVT patients. Methodology A complete stasis model was used for inducing thrombus in Sprague Dawley rats by ligating their Inferior Vena Cava (IVC). Animal experiments were divided into two groups: Ligated group (with IVC ligation) and SHAM Control group (without ligation). D‐Dimer and Prothrombin Fragment (PF) 1+2 levels were checked to confirm thrombus formation. Further, HDL‐associated proteins and other molecules were quantified using their respective ELISA kits. Similar tests were done in a cohort of human patients diagnosed with DVT (confirmed by a color Doppler test) and the results were compared with healthy individuals. All experiments were done in accordance with their respective ethical guidelines. Results D‐Dimer, PF 1+2 and inflammatory molecules were significantly higher in the ligated group, confirming the formation of thrombus. HDL associated proteins (APO‐A1, PON‐1, PAF‐AH & S‐1‐P) were significantly lower whereas oxidized lipoproteins (Ox‐HDL and Ox‐LDL) and lipooxygenase enzymes (15/12‐LOX) were found to be higher in the ligated group as compared to the SHAM control group. DVT patients showed a similar trend of D‐Dimer and PF 1+2 as in animals, when compared to healthy individuals. Lipid profile (Total cholesterol, Triglycerides, LDL and HDL) of patients did not show any significant changes albeit levels of HDL associated proteins were lower than those in healthy volunteers, wherein oxidized lipoproteins were significantly higher. A significance level (p‐value) of <0.05 was used for all the tests. Conclusion and Discussion Positive D‐Dimer test, high PF 1+2 and inflammatory molecules in DVT patients & in the ligated group of animals, confirm the biochemical changes that occur during thrombosis. Low levels of HDL associated proteins and high levels of lipooxygenase enzymes, responsible for lipoprotein oxidation (proven by high levels of oxidised lipoproteins) support the dysfunctional HDL hypothesis in case of VTE. These findings give more insight in understanding the pathophysiology of VTE and might help in disease prevention/diagnosis/treatment. CORRELATION BETWEEN PLATELET COUNT, D-DIMER AND DISEASE OUTCOME IN PATIENTS ADMITTED WITH COVID 19 DISEASE IN A TERTIARY CARE CENTRE

Corona virus disease 2019 (COVID 19) is a novel infectious disease which causes a spectrum of disease with respiratory and systemic inammation which leads to severe disease in 10 – 15 % of patients. Some patients develop a severe proinammatory state which can be associated with a unique coagulopathy leading to complications like ARDS and DIC. COVID 19 is a disease which lacks established laboratory markers available to evaluate illness severity and disease outcome. COVID 19 patients often have thrombocytopenia (5-41%) and appear to have increased platelet consumption, together with a corresponding increase in platelet production. The degree of D-dimer elevation positively correlates with morbidity in COVID 19 patients. Platelet count and D-dimer may be used as biomarkers to assess disease severity and may be useful for triaging patients and predicting the outcome of disease in resource limited settings.

Implications of COVID-19 on Thrombotic Profile of Severely Affected Patients

Coronavirus disease 2019 (COVID-19) is a novel viral disease that spread as a global pandemic in 2020 by infecting millions of people across the world. Its clinical prognosis is dependent on various coagulatory parameters since thrombotic events are frequently associated with infection severity.A total of 383 COVID-19 patients enrolled in Rajiv Gandhi Super Specialty Hospital, Delhi, India, were included in the present retrospective study. Patients were divided into three categories, severe (n = 141), moderate (n = 138), and mild (n = 104) based on infection severity. Various thrombotic parameters and anticoagulant levels were measured in 70 patients and further analyzed.Coagulopathy is seen in COVID-19 patients (n = 70) with a significant increase in fibrinogen, D-dimer levels, and prothrombin time in patients with severe and moderate disease compared to patients with a mild infection. Approximately, 70% of patients with severe and moderate disease demonstrated fibrinogen levels higher than the standard reference range. 60.41% of patients with severe disease showed significantly higher D-dimer levels. Thrombotic parameters were notably elevated in the nonsurvivors group compared to COVID-19 survivors. Nearly, 91% of patients with severe infection had anticoagulant protein S levels below the reference range.COVID-19 infection severely impacts the blood coagulation cascade, which might lead to the manifestation of severe symptoms and increased mortality in patients.

Pathophysiology of Venous Thromboembolism: Putative role of HDL

In 2019, cardiovascular disease (CVD) was the main reason of all global deaths (32%) and out of these deaths, 85% were due to heart attack and stroke alone. Venous thromboembolism (VTE), which is the third leading cause of CVD, has a very high incidence rate in Indian Population. Data suggest that thromboembolic disorders are due to multiple genetic and environmental variables and hence considered as a complex multifactorial trait. High Density Lipoprotein (HDL) has attracted a great deal of interest in recent years because of its implications in the prevention of various CVD. Various data suggests the relation between HDL and the expression of coagulation/fibrinolytic factors among the patients of CVD. Protein content of HDL is largely responsible for antioxidant mechanism by metabolizing lipid hydro peroxides and preventing their accumulation. Association of venous thrombosis and dyslipidemia has been investigated in several clinical studies, but results are inconsistent. Therefore, we sought to identify the role of HDL in pathophysiology of VTE in both human and animal studies.DVT was detected in male patients using color Doppler followed by D-Dimer and Prothrombin Fragment 1+2 (PF1+2) levels to confirm thrombus formation. HDL associated proteins, endothelial adhesion molecules, thrombotic molecules, inflammatory molecules and oxidized lipoprotein levels were measured. For animal study, a complete stasis animal model was standardized to generate thrombus formation in the SD rats. Thrombus length/weight was measured along with D-Dimer & PF 1+2 levels. HDL was isolated from plasma and was used in kinetic reaction of LDL oxidation. Progression of LDL oxidation was measured through DCFH-method. All experimental procedures were performed in accordance to ethical guidelines.A well written consent was taken from all the participants before conducting human study. High levels of D-Dimer and PF 1+2 in DVT patients as well as IVC ligated group confirmed thrombosis. HDL associated molecules were found to be significantly lower, whereas adhesion molecules, inflammatory molecules, and thrombotic molecules were significantly higher in both DVT patients and IVC ligated group. Oxidized lipoproteins and the lipooxygenase enzymes responsible for lipoprotein oxidation were found significantly higher. Isolated HDL used in LDL oxidation kinetics reaction found to be non-functional in preventing LDL oxidation. P-value of <0.05 was used as significance for all the tests.Low levels of HDL proteins along with high levels of adhesion molecules, thrombotic markers and inflammatory molecules indicate the suppression of HDL activity. High lipooxygenase enzymes and oxidized lipoprotein levels support the hypothesis of dysfunctional HDL. HDL isolated from the ligated group was unable to prevent the LDL oxidation which clarifies that HDL loses its functionality in case of VTE. Furthermore, finding of this study will also help in understanding the pathophysiology of VTE which might give an insight in prevention/early diagnosis of the disease.
