[Exploration of the early detection of lung parenchyma micronodules, nodule coalescence and emphysema by CT and HRCT in coal miners with and without coal-worker's pneumoconiosis evidence].

To explore the sensitivity and accuracy of CT and HRCT in early detection of coal-worker's pneumoconiosis (CWP) associated micronodules, nodule coalescence and emphysema from coal miners with and without radiographic CWP evidence.Continuously Thorax-Vol. CT scanning and High resolution CT scanning were performed on 113 coal miners with or without radiographic CWP evidence and 37 health controls by the use of Multi Spiral Computed Tomography (MSCT). The CT and HRCT images were evaluated for lung parenchyma opacity profusion category, nodule coalescence, emphysema index and mean lung attenuation, and then compared with that on radiographs.Good agreement for CWP associated opacity profusion grading was achieved between radiographs and CT scans (Kappa = 0.771). The sensitivity of CT and HRCT were 98.70% for CWP diagnosis when compared with radiographs. 8 (22.22%) cases out of 36 radiographic CWP negative coal miners were categorized as grade 1 opacity profusion in consistent with CWP on CT and HRCT scans, and 26 (35.62%) cases out of 73 radiographic grade 1-2 CWP patients were detected to have at least grade 3 nodular profusion on CT and HRCT scans. Among 113 coal miners, 36 (31.86%) were recognized as emphysema positive on CT; whereas only 7 (6.19%) on radiographs. According to CT opacity profusion grading, mean lung attenuation in coal miners with grade 1-2 opacity profusion classification was significantly increased than that in health control, CWP negative coal miner and grade 4 opacity profusion groups (F1 = -45.73, F2 = -23.00 and F3 = 52.72, P < 0.01 and P < 0.05).CT and HRCT acquired from MSCT showed high sensitive and accurate for the early detection of micronodules, nodule coalescence and emphysema in coal miners. It could be used as semi-quantitative and quantitative method in early diagnosis of CWP and its complications.

Head-and-Face Anthropometric Survey of Chinese Workers

Millions of workers in China rely on respirators and other personal protective equipment to reduce the risk of injury and occupational diseases. However, it has been >25 years since the first survey of facial dimensions for Chinese adults was published, and it has never been completely updated. Thus, an anthropometric survey of Chinese civilian workers was conducted in 2006. A total of 3000 subjects (2026 males and 974 females) between the ages of 18 and 66 years old was measured using traditional techniques. Nineteen facial dimensions, height, weight, neck circumference, waist circumference and hip circumference were measured. A stratified sampling plan of three age strata and two gender strata was implemented. Linear regression analysis was used to evaluate the possible effects of gender, age, occupation and body size on facial dimensions. The regression coefficients for gender indicated that for all anthropometric dimensions, males had significantly larger measurements than females. As body mass index increased, dimensions measured increased significantly. Construction workers and miners had significantly smaller measurements than individuals employed in healthcare or manufacturing for a majority of dimensions. Five representative indexes of facial dimension (face length, face width, nose protrusion, bigonial breadth and nasal root breadth) were selected based on correlation and cluster analysis of all dimensions. Through comparison with the facial dimensions of American subjects, this study indicated that Chinese civilian workers have shorter face length, smaller nose protrusion, larger face width and longer lip length.

Expression of Heat Stock Proteins in Cases of Lung Cancer of Coal Miners

[Objective] To explore the role of heat shock protein HSP27、HSP72 and HSP73 in the development of lung cancer in coal miner. [Methods] The total of 51 coal miners suffering from lung cancer was selected as cases and 99 healthy coal miners of the same age and employed in same coal mine were selected as controls. The expression of HSP27,HSP72 and HSP73 in peripheral blood lymphocytes were determined for all subjects. Possible risk factors of lung cancer were analyzed for these coal workers.[ Results] Compared to controls,both small and large stress-inducible HSP27(16.94±2.73)and HSP72(17.93±4.63) expressions of peripheral blood lymphocytes for lung cancer cases were significantly decreased than that in healthy coal miners (HSP27:22.94±10.21,HSP72:23.12±12.61,P 0.01). No statistic difference was observed for HSP73 in the two groups. The risk factors for lung cancer were found to be high cumulative coal dust exposure, the amount of smoking and decreased HSP27. [Conclusion] Long and high exposure to coal dust could reduce stress response ability. The HSP27 and HSP72 expression in peripheral blood lymphocytes decreased in coal miners who developed lung cancer. HSP27 may play a crucial role in prediction of coal dust over-loading as well as lung cancer development.

We also thank Dr. Cappello and his colleagues for raising these important issues concerning the plasma levels of Hsp27 in people with chronic obstructive pulmonary disease (COPD).In our study, we found that plasma Hsp27 levels in COPD patients with or without pneumoconiosis were lower than those in the controls (Cui et al. 2015), which was not in agreement with the study by Dr. Cappello and his colleagues (2011).They found no difference in the histological levels of Hsp27 when comparing COPD patients with control patients.When they combined all COPD patients (mild/moderate COPD and severe/very severe COPD) together, they observed that Hsp27 levels in bronchial mucosa of all COPD patients were higher than those of non-smoker controls.Several reasons might contribute to the difference between our results and conclusions and those of Dr. Cappello and his colleagues (2011).Firstly, all COPD patients and controls in our study were coal mine workers.

The feature and significance of increased concentration of SP-A and TNF-α in BALF of early stage pneumoconiosis

Objective To explore the feature and significance of releasing cytokines from alveolar macrophages and type Ⅱ cells in coal miners after exposure to coal dust or with early pneumoconiosis. Methods 23 coal miners were selected and divided into 2 groups according to their X ray expressions,i.e. exposure to coal dust group,0 + and Ⅰ stage group,and 7 healthy smoking farmers as control group.BALF was collected with fibrobronchoendoscope,then the concentrations of surface protein A of alveoli(SP A) and tumor necrosis factor α(TNF α) in the BALF were measured with ELISA. Results The mean value of SP A was (6 528.2±1 872.0)ng/ml,and TNF α was (8.23±1.61)U/ml in the exposure group,while those in control were (1 050.1±912.2)ng/ml and (5.17±0.89)U/ml,respectively.There were statistical differences of the concentrations of SP A and TNF α between the two groups( P 0.01).Moreover,the concentrations of SP A and TNF α showed a decreasing trend with the development of pneumoconiosis stage( r =-0.64, P 0.01; r =-0.45, P 0.05,respectively). Conclusion The increasing concentrations of SP A and TNF α in BALF may be effective markers for diagnosing coal dust exposure.

Expression Levels of Surfactant-Associated Proteins and Inflammation Cytokines in Serum and Bronchoalveolar Lavage Fluid Among Coal Miners

Objective: To investigate whether the Th1/Th2 balance and expressions of surfactant-associated proteins and cytokines in serum and bronchoalveolar lavage fluid (BALF) are associated with the development of coal workers' pneumoconiosis (CWP). Methods: A case-control study was conducted among 72 CWP cases and 68 controls. Th1 and Th2 populations were measured by flow cytometry. Expressions of surfactant-associated proteins A and D (SPA and SPD) and cytokines in serum and BALF were detected by enzyme-linked-immunosorbent serologic assay. Data were analyzed by t test and logistic regression. Results: Higher Th2 and lower Th1/Th2 were observed in CWP (P < 0.05). Increased CWP risk was associated with elevated BALF-interleukin-10 (odds ratio [OR]: 25.97; 95% confidence interval [CI]: 2.17 to 49.77), serum-SPA/BALF-SPA (OR: 12.87; 95% CI: 1.11 to 148.72), and serum-SPD/BALF-SPD (OR: 77.91; 95% CI: 5.64 to 161.46), decreased BALF-SPA (OR: 0.06; 95% CI: 0.01 to 0.31) and BALF-SPD (OR: 0.07; 95% CI: 0.02 to 0.36). Conclusions: The development of CWP might be associated with Th1/Th2 imbalance and increased BALF-interleukin-10, serum-SPA/BALF-SPA, and serum-SPD/BALF-SPD.

COPD and levels of Hsp70 (HSPA1A) and Hsp27 (HSPB1) in plasma and lymphocytes among coal workers: a case-control study

This case-control study aimed to investigate whether the levels of Hsp70 (HSPA1A) and Hsp27 (HSPB1) in plasma and lymphocytes were associated with the risk of chronic obstructive pulmonary disease (COPD) among coal workers. A total of 76 COPD cases and 48 age-matched healthy controls from a group of coal workers were included. The case group consisted of 35 COPD patients whose condition was complicated with coal workers' pneumoconiosis (CWP) and 41 COPD patients without CWP. Heat shock proteins (Hsps) in plasma and lymphocytes were detected by ELISA and flow cytometry, respectively. Multiple logistic regression models were applied to estimate the association between Hsp levels and COPD risk. Our results showed that plasma Hsp70 and lymphocyte Hsp27 levels were significantly higher and plasma Hsp27 levels were significantly lower in COPD cases than in controls (p < 0.01). No significant differences in lymphocyte Hsp70 levels were found between COPD cases and the matched subjects. Higher plasma Hsp70 levels (odds ratio (OR) = 13.8, 95 % confidence interval (CI) = 5.7-33.5) and lower plasma Hsp27 levels (OR = 4.6, 95 % CI = 2.0-10.5) were significantly associated with an increased risk of COPD after adjusting for confounders. Higher lymphocyte Hsp27 levels were only associated with an increased risk of COPD with CWP (OR = 6.6, 95 % CI = 2.0-22.1) but not with an increased risk of COPD without CWP (OR = 3.0, 95 % CI = 0.9-8.9). Additionally, there were strong joint effects of different Hsps on COPD risk. These results showed that higher levels of plasma Hsp70 and lower levels of plasma Hsp27 might be associated with an increased risk of COPD among coal workers. They may have the potential to serve as monitoring markers for COPD in coal workers.

X线胸片是诊断尘肺的主要临床依据,我国第1版尘肺诊断标准1963年正式实施,其后于1986年进行了修订,使用16年后,2002年进行再次修订(1997年国家又发布了一版诊断标准,由于该标准的核心部分与1986年标准相比并没有改动,因此人们习惯上仍称为86标准).随着社会对尘肺诊断准确性的需求,2009年又颁布了修订的尘肺诊断标准.这些标准均为尘肺诊断提供了技术层面的指南[1-5].为全面了解我国尘肺病诊断标准的变化情况,以及各版标准诊断之间的异同(表1),我们对各版尘肺诊断标准的异同进行了汇总。
