Physical Activity, Genetic Susceptibility, and Risk of Colorectal Cancer in Type 2 Diabetes: A Large Population-Based Cohort Study.

It is crucial to identify modifiable preventive measures to mitigate the risk of colorectal cancer (CRC) among patients with type 2 diabetes (T2D), a high-risk group for CRC. We conducted a study to investigate the potential association between various levels of physical activity (PA) and the incidence of CRC, taking account of genetic susceptibility, in a T2D population cohort. The study was based on UK Biobank (UKB) data on persons diagnosed with T2D; the participants were tracked until 2022. Hazard ratios (HR) and 95% confidence intervals (95% CI) for CRC were calculated using Cox regression models. The 33 733 patients with T2D were followed up for a median of 13.62 years. During this time, 551 patients were diagnosed with CRC. Compared with low PA, the multivariable adjusted HR for CRC among T2D patients was 0.81 (95% CI [0.66; 0.98]) and 0.74 (95% CI [0.58; 0.94]) in the groups with moderate and high PA, respectively. In right colon cancer, moderate and high levels of PA were associated with 31% (HR 0.69; 95% CI [0.51; 0.93]) and 42% (HR 0.58; 95% CI [0.40: 0.85]) reductions in the risk of CRC. High PA (HR 0.54; 95% CI, 0.35-0.84) was associated with a lower risk of CRC even in patients with a high polygenic risk score (PRS). Persons with low PRS and high PA had the lowest risk of CRC. Our study suggests that moderate to high PA helps to reduce the risk of CRC in T2D patients and that joint consideration of PA level and PRS could provide valuable insights for personalized strategies to prevent CRC.

Bromodomain 4 is a potent prognostic marker associated with immune cell infiltration in breast cancer

Abstract Bromodomain 4 (BRD4), a member of the bromodomain and extra‐terminal domain protein family, has become a promising epigenetic target in cancer and inflammatory diseases; however, the detailed biological role of BRD4 in breast cancer (BRCA) remains undetermined. We analysed the BRD4 expression levels using the Oncomine and TIMER databases and evaluated the clinical impact of BRD4 on BRCA prognosis using Kaplan‐Meier plot and PrognoScan. The correlation between BRD4 and tumour‐infiltrating immune cells was investigated using TIMER. Furthermore, the correlation between BRD4 expression levels was also analysed using TIMER in addition to the GEPIA database for immune cell gene markers. BRD4 expression was significantly higher in BRCA tissues than in normal tissues, which was significantly correlated with poor overall survival (OS). Specifically, high BRD4 expression was correlated with worse OS and progression‐free survival in patients with BRCA. In addition, BRD4 expression was correlated with levels of infiltrating monocytes (CSF1R, cor = 0.204, P = 9.19e−12), tumour‐associated macrophages (CD68, cor = 0.129, P = 1.81e−05), M1/M2 macrophages and different effector T cells (including Th1/Th2/Treg) in BRCA. These findings suggest that BRD4 could be used as a prognostic biomarker for determining prognosis and immune cell infiltration levels in BRCA.

Effect of ethyl pyruvate on expression of pulmonary high mobility group box-1 protein and acute lung injury in rats with delayed resuscitation after burn injury

Objective To investigate the effect of ethyl pyruvate(EP) on pulmonary high mobility group box-1 protein(HMGB1) expression and acute lung injury in rats with delayed resuscitation after burn injury.Methods Wistar rats were inflicted to 30% full-thickness scald injury followed with delayed resuscitation.Seventy-eight male rats were randomly divided into sham scald group(n=18),scald group(n=30),and EP treatment group(n=30),and they were sacrificed at 8,24,and 72 h after scald injury.Lung tissue samples were collected to determine HMGB1 mRNA as well as protein expressions,and pulmonary myeloperoxidase(MPO) activity.HMGB1 mRNA level was semi-quantitatively measured by the reverse transcription polymerase chain reaction(RT-PCR) taking GAPDH as an internal standard,and protein expression of HMGB1 was detected by Western blotting and immunohistochemistry.The pathological changes of lung tissues were observed with light microscopy after HE staining.Results Compared to sham scald controls,both mRNA and protein expressions of HMGB1 were significantly enhanced in the lung at 8 to 72 h after scald injury(P0.05 or 0.01),meanwhile pulmonary MPO activities were markedly increased at 8 and 24 h after scald(P0.01).In addition,many inflammatory cells in pulmonary tissues were observed under light microscope following injury.Treatment with EP markedly down-regulated pulmonary HMGB1 mRNA and protein expressions at 8 to 72 h(P0.05 or 0.01),and reduced pulmonary MPO activities at 8 and 24 h following scald injury(P0.01).The histological morphology of pulmonary lesions was ameliorated after treatment with EP.Conclusion These results suggest that HMGB1 might act as a late mediator in the pathogenesis of postburn acute lung injury.Treatment with EP could obviously inhibit pulmonary HMGB1 expression and reduce MPO activity,thereby prevent the development of acute lung injury induced by major burns with delayed resuscitation.

Monocytic myeloid-derived suppressor cells contribute to the exacerbation of bone destruction in periodontitis

Periodontitis (PD) is a chronic infectious and inflammatory disease characterized by alveolar bone loss. The distinctive activity of immune cells critically exacerbates bone resorption in PD. Myeloid-derived suppressor cells (MDSCs) are known to contribute to various chronic inflammatory conditions, but their role in the pathogenesis and progression of PD remains poorly understood. We used single-cell transcriptomic analysis with human gingival samples and animal models of experimental periodontitis to examine the role of M-MDSCs in PD. We also explored the therapeutic effect of depleting MDSCs on PD in vivo. Additionally, the mechanisms of long non-coding RNA Neat1 and the pathway of NF-κB-dependent "canonical NLRP3 inflammasome activation" in MDSCs were investigated in PD. In this study, we revealed that monocytic (M)-MDSCs were significantly increased in inflamed gingiva of PD patients compared to healthy individuals. Expansion of M-MDSCs was also observed in the mouse model of ligature-induced periodontitis, and depletion of MDSCs in PD mice could ameliorate alveolar bone loss and reduce periodontal inflammation. Mechanistically, we found that long non-coding RNA Neat1 was significantly upregulated in M-MDSCs, which achieved this proinflammatory effect by activating NF-κB signaling in PD. Furthermore, the pathway of NF-κB-dependent "canonical NLRP3 inflammasome activation" was confirmed in the PD mouse model, accompanied by increased secretion of proinflammatory cytokines that drive alveolar bone loss, including IL-1β, IL-6 and TNF-α. In conclusion, this study highlights the pivotal proinflammatory role of M-MDSCs in PD and suggests that targeting these cells may represent a novel immunotherapeutic approach. Future research could focus on strategies to specifically target MDSCs for the treatment of periodontitis.

Effect of self-made Runfei Jianpi Yishen Decoction on pulmonary function and inflammatory factors in children with refractory asthma

Objective To explore the effect of self-made Runfei Jianpi Yishen Decoctionin in the treatment of refractory asthma in children, and its influence on pulmonary function, inflammatory factors and clinical symptom score. Methods 88 children with refractory asthma admitted to our hospital from February 2015 to December 2016 were retrospectively selected and divided into control group and observation group according to different treatment methods. The control group was treated with routine western medicine, while the observation group was treated with self-made Runfei Jianpi Yishen Decoction on the basis of the control group. The changes of pulmonary function, clinical symptom score and cytokine levels in two groups were observed. Results The scores of clinical symptoms in the two groups were improved after treatment, and those in the observation group were better than those in the control group (P<0.05). After treatment, the levels of forced vital capacity (FVC), forced expiratory volume in one second (FEV1) and peak expiratory flow (PEF) in the two groups were higher than those before treatment. The levels of FVC, FEV1 and PEF in the observation group were higher than those in the control group (P<0.05). After treatment, the levels of interleukin (IL)-8, IgE, osteopontin (OPN) and eosinophil cationic protein (ECP) in the two groups were decreased, while the levels of interferon (IFN)-γ and Cor were increased (P<0.05). After treatment, the levels of IL-8, IgE, OPN and ECP in the observation group were lower than those in the control group, and the levels of IFN-γ and Cor in the observation group were higher than those in the control group (P<0.05). Conclusions The self-made Runfei Jianpi Yishen Decoctionin is effective in the treatment of refractory asthma in children. It can effectively improve the pulmonary function and regulate the levels of immune factors, OPN and ECP in children. Ferroptosis Patterns Correlate with Immune Microenvironment Characterization in Gastric Cancer

We aimed to build a ferroptosis-based classifier to characterize the molecular features of gastric cancers (GC) and investigate the relationship between different ferroptosis patterns and GC tumor microenvironment (TME).Based on the genomic and clinical information from TCGA portal and GEO database, non-negative matrix factorization (NMF) was used to identify ferroptosis subtypes in GC patients. In order to estimate the ferroptosis levels, we established ferroptosis subtype score (FSS) to quantify ferroptosis patterns and ferroptosis potential index (FPI) by principal component analysis (PCA). The correlations of different ferroptosis patterns with TME cell-infiltrating characteristics (including immune cell infiltration, immune checkpoints expression levels, tumor mutational burden (TMB) and immunotherapy response) were systematically analyzed.Two ferroptosis subtypes, C1 (with lower FSS) and C2 (with higher FSS), were determined. C2 displayed a significantly lower FPI than C1. Besides, C2 was associated with diffuse subtype while C1 with intestinal subtype. As for TME characteristics, C2 was in accordance with the immune-excluded phenotype as it showed more active immune and stromal activities but lower TMB, less probability of immunotherapy response and poorer prognosis. C1 was linked to immune-inflamed phenotype as it had lower stromal activities but increased neoantigen load, enhanced response to immunotherapy and relatively better prognosis.The systematic assessment of ferroptosis patterns and ferroptosis levels presented in our study implied that ferroptosis serves as an important factor in the formation of TME, which may expand the understanding of TME and provide a novel perspective for the development of targeted immunotherapeutic strategies for GC patients.

Rutin ameliorates cigarette smoke mediated lung inflammation in a mouse model of chronic obstructive pulmonary disease via regulating the levels of tumor necrosis factor-α, interleukin-8 and PAF

Introduction Rutin is a plant-derived flavonoid. In the present work we screened the activity of rutin in a cigarette smoke induced lung disease mouse model of chronic obstructive pulmonary disease (COPD). Material and methods The mice were subjected to cigarette smoke for two weeks for inducing COPD simulating lung inflammation. Two hours prior to exposure to cigarette smoke, the mice were treated with rutin via the oral route. After treatment, the mice were assessed for profiling of cytokines and infiltration of immune cells in the bronchoalveolar fluid and histological alterations in the lungs. Roflumilast, an anti-COPD drug, was used as a positive control for comparing the efficacy of rutin. Results The results of the study suggested that the treatment with rutin inhibited the inflow of macrophages, lymphocytes and neutrophils significantly in the airway. It was also observed that the treatment with rutin significantly decreased tumor necrosis factor-, interleukin-8 and platelet activating factor levels in the bronchoalveolar lavage fluid compared to the cigarette smoke exposed group. Interestingly, the findings of our study were parallel to the positive control RFL included in the experiment in the treated mice. Conclusions The findings suggested that rutin exerts a protective effect against cigarette smoke mediated COPD-like lung inflammation. Hence rutin may be a novel therapeutic agent for treating inflammatory conditions in lungs such as COPD.

Downregulation of MerTK in circulating T cells of patients with non-proliferative diabetic retinopathy

To explore the differential gene expression in peripheral blood immune cells of individuals with type 2 diabetes mellitus (DM), comparing those with and without non-proliferative diabetic retinopathy (NPDR). From a pool of 126 potential participants, 60 were selected for detailed analysis. This group included 12 healthy donors (HDs), 22 individuals with DM, and 26 with NPDR. We analyzed peripheral blood mononuclear cells (PBMCs) using RNA sequencing and quantitative PCR (qPCR) to pinpoint differentially expressed genes (DEGs). Western blot and flow cytometry were also employed to evaluate the protein expression of specific genes. In patients with NPDR compared to those with DM alone, MerTK-a gene implicated in inherited retinal dystrophies due to its mutations-was notably downregulated in PBMCs. Through flow cytometry, we assessed the protein levels and cellular distribution of MerTK, finding a predominant expression in monocytes and myeloid-derived suppressor cells (MDSCs), with a marked reduction in CD4+ and CD8+ T cells, as well as in natural killer T (NKT) cells. Patients with DM demonstrated a significant deviation in the PBMCs composition, particularly in B cells, CD4+ T cells, and NK cells, when compared to HDs. The study indicates that MerTK expression in T cells within PBMCs could act as a viable blood biomarker for NPDR risk in patients with DM. Furthermore, the regulation of T cells by MerTK might represent a critical pathway through which DM evolves into NPDR.

[Effect of Dexmedetomidine Combined Electrical Stimulation on Coanitive Function of Patients Receiving Extracerebral Intervention].

To explore the effect of dexmedetomidine combined electrical stimulation on cognitive function of neurosurgical diseases patients treated by extracerebral intervention.Totally 122 patients with neurosurgical diseases who underwent selective intervention were randomly assigned to the observation group and the control group, 61 cases in each group. Patients in the control group recieved anesthesia by dexmedetomidine. Those in the observation group received electrical stimulation at Baihui (DU20), Yintang ( EX-HN3), and Neiguan (PC6) before dexmedetomidine anesthesia. The cognitive function of patients at preoperative day 1 and postoperative day 1 was respectively evaluated by Mini-Mental State Examinations (MMSE). Serum NSE, S-100β, IL-1β, IL-6, and TNF-α levels were detected in the two groups before intervention and immediately after intervention using ELISA.MMSE scores of two groups were significantly reduced at post-intervention day 1, as compared with one day before intervention. MMSE score of the observation group at post-intervention day 1 was (23.15 ± 1.87) points, significantly higher than that of the control group [ (19.34 ± 1.64) points , (P < 0.05)]. The postoperative cognitive dysfunction (POCD) incidence rate of the observation group was 16.4% (10/61), significantly lower than that of the control group [39.3% (24/61); P < 0.05]. Compared with before intervention, NSE and S-100β protein levels, IL-1β, IL-6 and α-TNF levels of the two groups increased (P < 0.05). Post-intervention NSE and S-100β protein levels, IL-1β, IL-6 and α-TNF levels were significantly lower in the observation group than in the control group (P < 0.05).Dexmedetomidine combied electrical stimulation could effectively prevent the occurrence of postoperative cognition, and reduce levels of NSA, S-100β, IL-1β, IL-6 and TNF-α.
