Reduction of Ischemic Injury by Nitroglycerin during Acute Myocardial Infarction

ARRHYTHMIAS and cardiac failure are two of the major, potentially lethal sequelae of acute myocardial infarction. Although measures used to treat arrhythmias are usually considered separately from those used to treat myocardial failure, it is clear that arrhythmias and myocardial failure share a common precipitating mechanism —severe ischemic injury (as defined by a reduction in oxygen delivery to the myocardial cell resulting initially in aberrations of biochemical, electrophysiologic and mechanical function and, if unrelieved, ultimately in cell death). Postoperative Assessment of Patients with Ventricular Septal Defect and Pulmonary Hypertension

Circulatory function of patients with operatively closed ventricular septal defect and preoperative pulmonary arterial hypertension was evaluated in 11 asymptomatic subjects at rest and during intense treadmill exercise three to 15 years after operation. The pulmonary-to-systemic resistance ratio was normal or mildly to moderately elevated preoperatively. Postoperative mean pulmonary arterial pressure at the time of study was normal or mildly elevated at rest in 10 patients and moderately elevated in one (40 mm Hg). During intense upright exercise sufficient to lower pulmonary arterial oxygen saturation to 30%, cardiac output was below the normal range in five patients. Each of these patients had been operated upon after 10 years of age. The magnitude of the postoperative abnormality in cardiac output response to exercise was directly related to age at operation. Two of the five patients with impaired cardiac output response and two other patients manifested an abnormally elevated mean pulmonary arterial pressure during intense exercise. There was a positive correlation between pulmonary arterial pressure during intense exercise and age at operation. These results indicate that late postoperative cardiovascular function may be abnormal in patients with ventricular septal defect and preoperative pulmonary arterial hypertension, and that these abnormalities appear to be related to age at operation. Since all patients were asymptomatic, the long-term clinical significance of these hemodynamic abnormalities remains to be determined.

Effect of prolonged space flight on cardiac function and dimensions

By taking advantage of the capabilities of echocardiography to measure noninvasively left ventricular volume, stroke volume, and ejection fraction, and of the fact that the astronauts were routinely subjected to lower body negative pressure (whereby cardiac filling is progressively decreased), it was possible to construct classic ventricular function curves noninvasively, thereby obviating the difficulties encountered in comparing cardiac function at different end-diastolic volumes preflight and postflight. In this manner, the effect of an 84-day period of weightlessness on cardiac structure and function was evaluated in the Skylab 4 astronauts.

Treatment of Angina Pectoris by Electrical Stimulation of the Carotid-Sinus Nerves

Radiofrequency carotid-sinus nerve stimulators implanted for treatment of incapacitating angina pectoris, unrelieved by intensive medical management, produced appreciable symptomatic improvement in 13 of 17 patients: nearly all episodes of angina were terminated by activation of the stimulator, and prophylactic use increased intensity and duration of exercise that could be performed. After long-term use exercise capacity was increased in many patients even when the stimulator was not activated. Two had no benefit, and two died. Prophylactic use during exercise lowered mean arterial pressure an average of 18 mm of mercury (p less than 0.001) in 13 of the 15 survivors, but heart rate was reduced by an average of only 4 beats per minute. The other two showed no hemodynamic response. The principal physiologic factor in the relief of angina appears to be a decrease in arterial pressure, which reduces a major determinant of myocardial oxygen consumption and re-establishes the balance between myocardial oxygen demands and supply.
