Intrapleural immunotherapy for metastatic pleurisies demonstrates a high efficiency in the treatment of patients with breast cancer (BC). This immunotherapy modality is regarded as one of the stages of complex treatment in patients with disseminated BC and allows its capabilities to be extended for their further management.

In this study we have examined the immune response in cancer patients treated with dendritic cell vaccine. Antitumor response was estimated by ELISPOT method, one of the basic methods determining the number of cytokineproduced cells. Lymphocytes were incubated with phytohaemagglutinin (PHA) and tumor lysate. 14 patients were enrolled in this study. It was determined that the number of interferon-γ secreted cells increased few-fold in 8 patients during the vaccination, the number of interferon-γ secreted cells reduced in 5 patients and there was no response in one patient. Moreover, we have observed that the increasing of immune response in 4 patients was correlated with the response to PHA and the decreasing of immune response was correlated with the decrease of immune response to PHA in one patient. Based on data obtained we conclude that the dendritic vaccine is effective.

In this study we have estimated the serum level of S-100 marker in patients with disseminated melanoma during dendritic cell vaccination. Before vaccinotherapy, the patients were divided into two subgroups with high and low level of S-100 marker in compare with healthy donors. Median survival of patients with high level of S-100 marker were 22,0±7,8 months, whereas in group with low level of S-100 marker median survival were 54±25,2 months (p=0,045 ). We have observed that S-100 marker level increased in less degree in seum of patients with initial high level of soluble MHC I class molecule and high level of soluble CD8 during dendritic cell vaccination. The data obtained indicate that the alteration in MHC I class molecule and CD8 antigen level in patents with disseminated melanoma treated with dendritic vaccine could be considered of prognostic significance.

The aim of this study was to examine the safety and prognosis of side and toxic effects of anticancer vaccine «Melovac». We have studied the local and systemic toxicity, its ability to be allergic and tumorogenic. Based on our pre-clinical study which reveal that «Melovac» vaccine is not toxic, we recommend it for clinical trials.
