Effect of clavulanic acid and amoxycillin formulation against β-lactamase producing Gram-negative bacteria in urinary tract infections

Six patients with urinary tract infections due to β-lactamase producing Gram-negative bacteria received a single dose of amoxycillin (500 mg), followed 48 h later by a single dose of the formulation clavulanic acid (100 mg) and amoxycillin (500 mg). Bacterial counts and antibiotic assay in the urine were undertaken at 0,4,8,24 and 48 h. In the presence of clavulanic acid, the urine level of amoxycillin achieved a normal range and the bacterial count dramatically decreased.

Dissemination of the novel plasmid-mediated beta-lactamase CTX-1, which confers resistance to broad-spectrum cephalosporins, and its inhibition by beta-lactamase inhibitors

The novel beta-lactamase CTX-1 (pI 6.3) encoded on a transferable 84-kilobase plasmid was found in six different bacterial species. It was responsible for a significant decrease in susceptibility towards most penicillins and cephalosporins, except imipenem, temocillin, and cephalosporins which have a 7-alpha-methoxy substituent. Synergy between either ampicillin, piperacillin, cefotaxime, ceftazidime, or aztreonam and three beta-lactamase inhibitors (clavulanic acid, sulbactam, and YTR 830) was generally found for different strains harboring CTX-1. This enzyme may be related to or derived from the TEM enzyme, since an intragenic probe of the TEM-1 gene hybridized with a fragment of the plasmid carrying CTX-1.

SYN987, SYN1193, and SYN1253, new quinolones highly active against Gram-positive cocci

SYN987, 1193 and 1253 are new fluorinated quinolones with enhanced activity against Gram-positive bacteria. The MICs of SYN 987, 1193 or 1253 were studied by an agar dilution method against 88 streptococci (including penicillin resistant Streptococcus pneumoniae), 30 enterococci and 62 Staphylococcus aureus. SYN 987, 1193 and 1253 were 2 to 4 times more active than sparfloxacin and 16-64 times more active than older fluoroquinolones against all streptococci and ciprofloxacin-resistant S. aureus. This study demonstrates that SYN 987, 1193 and 1253 are extremely potent antibacterial agents which deserve further evaluation.

Activity of the β-lactamase inhibitor BRL 42715 against cephalosporinases produced by Enterobacteriaceae

BRL 42715, a novel beta-lactamase inhibitor, was evaluated for its capacity to inhibit cephalosporinases. BRL 42715 was effective in potentiating the activity of antibiotics against clinical isolates of Enterobacteriaceae that produced high levels of cephalosporinases. This correlated well with the very low 50% inhibition values (< 0.004 mg/L) of BRL 42715 for cephalosporinases extracted from different species. When compared in vitro to clavulanic acid, sulbactam, and tazobactam, BRL 42715 was the most efficient inhibitor of cephalosporinase.
