Eosinophilic cellulitis - A case study and management with griseofulvin.

A 47 year old hypertensive and diabetic male patient suffering from eosinophilic cellulitis (Wells syndrome) is being reported. The patient responded very well to oral griseofulvin at the time of first presentation and subsequent three recurrences in a period of three years.

We report a two and a half year old male child with dystrophy of all the nails, absent patellae and iliac horns. In addition he had dysmorphic facial features, sparing of lunula and bilateral hallux valgus deformity, hitherto not reported earlier.

Epidemic dropsy: a study of cutaneous manifestations with histopathological correlation.

Clinical and histopathological features of epidemic dropsy were studied in 19 patients. Bilateral pitting pedal oedema, erythrocyanosis and tachycardia without fever were the predominant clinical features. Histology revealed deposition of hyaline material on the walls of dermal blood vessels.

PUVASOL therapy in alopecia areata

Thirty two patients having alopecia areata of less than two months duration were inducted into the study. This included 20 males and 12 females. Of the 28 cases who completed the study, 20 (71.42%) showed complete hair regrowth in 14 weeks, out of which 16 (57.14%) had it in 10 weeks and 4 (14.28%) in 14 weeks, 8 methoxy - psoralen (0.75% solution) was applied half an hour prior to the exposure to sunlight, for 30 seconds which was gradually increased to 2 minutes. The procedure was repeated daily. No side effects were observed during the trial.

Liver function and immunoglobulins in skin lichen palnus

Fifty-two patients of skin lichen planus conforming to the clinical and histopathological description were subjected to the different liver function tests. Age and sex matched clinically normal equal number of persons were taken as controls to exclude liver disorders.] Seven (13.46%) of the patients used to consume mild to moderate amount of alcohol. Nine (17.30%) cases gave the history of jaundice in the past and all of them had hepatomegaly. Otherwise hepatomegaly was observed in 37 (71.15%) cases. None of the healthy controls had hepatomegaly. Aspartase iminotransferase (AST) alanine aminotransferase (ALT) and alkaline phosphatase was raised in 11 (21.15%), 4 (7.69%) and 10 (19.23%) cases respectively. HBsAg was not detected in any of the 52 patients.The most contradictory finding was the revelation of raised immunoglo bulin of G, A or M either singly or in different combinations. IgG and IgA were raised in 32 (61.53%) cases while IgM in 26 (50.0%).

Oral ulcer as an unusual feature of visceral leishmaniasis in an AIDS patient

Leishmaniasis, a globally prevalent parasitic disease, occurs in three forms, viz, visceral, cutaneous and mucocutaneous. It is transmitted by female Phlebotomus sandflies. Human immunodeficiency virus (HIV) infection is increasing worldwide and several reports indicate a rising trend of VL / HIV co-infection, modifying the traditional anthroponotic pattern of VL transmission. India is one of the countries having the largest burden of leishmaniasis; nevertheless, there are very few HIV / leishmania co-infection cases reported so far. We report a 35-year-old homemaker infected with the human immunodeficiency virus; she presented with an oral ulcer. The investigations carried out on her revealed that she was afflicted by visceral leishmaniasis and the oral ulceration was a part of the same. This is only the second such case from the Indian subcontinent and more significantly from a non-endemic area.

Sensitivity to Immune Checkpoint Blockade and Progression-Free Survival is associated with baseline CD8⁺ T cell clone size and cytotoxicity

ABSTRACT Immune checkpoint blockers (ICB) exert their anti-cancer effects via CD8 + T cells, although how responses vary over sub-populations and across clones is incompletely understood. We performed single-cell RNA-sequencing of CD8 + T cells and their receptors pre- and post-ICB across eight patients, integrating results with bulk-sequencing data (n=209). We identify seven subsets with divergent responses to ICB, finding the effector cluster demonstrates the most pronounced changes. Likewise, transcriptomic response to ICB relates to clone size, with large clones demonstrating increased numbers of regulated genes of higher immunological pertinence. Cytotoxic effector clones were more likely to persist long-term following ICB and overlapped with public tumour-infiltrating lymphocyte clonotypes. Notably, pre-treatment CD8 + cytotoxicity associated with progression-free survival, highlighting the importance of the baseline CD8 + immune landscape in long-term response. This work further advances understanding of the molecular determinants of ICB response and assists in the search for peripheral prognostic biomarkers. ONE SENTENCE SUMMARY Using single-cell and bulk RNA sequencing we explore checkpoint immunotherapy activity on peripheral CD8 + T cells in metastatic melanoma; demonstrating that cell subset and clone size determine gene expression responses to treatment, and that pre-treatment cytotoxicity and clonality of peripheral CD8 + T cells is clinically prognostic.
