The dwindling number of organ donors is a major concem in transplantation.'As a result there has been a renewed interest in retrieving kidneys from asystolic or "non-heart beating" donors using in situ renal perfusion.'3This report details our experience and results after one year in Leicester following the introduction of a dedicated programme to retrieve kidneys from non-heart beating donors.

While the radiological appearance of a large bowel fluid level is commonly associated with obstruction, we report a case which occurred acutely and was associated with unusual X-ray findings. A 42-year-old electrician presented to the Accident and Emergency Department with a 6 h history of lower abdominal pain.

Anti-Rejection Therapy in Renal Transplantation: Use of the Monoclonal Antibody OKT3

The success of anti-rejection therapy was evaluated in 65 cadaveric renal allografts immunosuppressed with cyclosporin. All first rejection episodes were treated with 0.5 g methylprednisolone given daily for 3 days. Second rejection episodes were treated with either a further identical 3-day course of methylprednisolone or a 10-14-day course of the monoclonal antibody OKT3. The peak creatinine during second rejection episodes was higher in patients treated with OKT3 (481 +/- 54 mumol/l; mean +/-SEM) than in those treated with methylprednisolone (320 +/- 59 mumol/l) (P = 0.036), suggesting more severe rejection in the OKT3 group. Despite this the rejection reversal rate was higher in the OKT3-treated group (100%) when compared to the high-dose methylprednisolone group (64%) (P less than 0.05) and the creatinine at 6 months was less in the OKT3 group (162 +/- 17 mumol/l) than in the methylprednisolone group (230 +/- 22 mumol/l). These data suggest that OKT3 is a very effective treatment for second rejection episodes following renal transplantation and yields improved renal functions in comparison with high-dose steroids.

Probability of rejection predicted from ultrasonographic measurement of renal transplant swelling

Abstract Serial ultrasonographic measurements of renal transplant crosssectional area were used to quantify allograft swelling as a diagnostic test of acute rejection. Eighty consecutive episodes of acute allograft dysfunction (rise in creatinine level ⩾ 10 per cent or ⩾ 30 μmol l−1) were investigated. Needle core biopsy was performed in all cases to diagnose acute rejection. Rejecting transplants demonstrated a median (interquartile range) swelling of 16·2 (12·1–25·5) per cent compared with 2·2 (−2·0 to 8·4) per cent for non-rejecting organs (P < 0·001). A ⩾ 10 per cent increase as the diagnostic threshold for rejection yielded a sensitivity of 80 per cent, specificity of 77 per cent, positive predictive value of 85 per cent, negative predictive value of 71 per cent and overall accuracy of 79 per cent. Using a logistic regression model, predictive probabilities of rejection for individual changes in cross-sectional area were calculated. A 20 per cent increase was associated with a predicted probability of rejection of 87 per cent (95 per cent confidence interval 75–94 per cent). There was a significant correlation between the functional severity of rejection and the degree of transplant swelling (rs = 0·63, P < 0·001). Ultrasonographic measurement of renal transplant cross-sectional area is a simple non-invasive test for the diagnosis of acute rejection.
