Design Variations in Double Oblique Device for Osteosynthesis (DODO) of Hip: A Novel Implant for the Northeast Population of India

Intertrochanteric fractures account for 50% of all hip fractures. Operative interventions include extramedullary or intramedullary fixation devices. One design of extramedullary fixation device, DODO (Double Oblique Device for Osteosynthesis), was proposed by Nag et al. [1]. The variation in size and shape of that implant is investigated in the present study. Also, the present study varied the angle of the distal-most screw of the proposed implant to check the plate lift-off phenomenon. The result of this study is validated against existing experimental studies on screw pull-out tests. Finite Element (FE) analysis is employed to compare the biomechanical stability of the femoral construct on an osteoporotic bone with that of a healthy patient bone. 3D CAD models of implants were designed and FE analysis was employed to compare the biomechanical outcome of newer proposed models with the base model. Realistic physiological load regime and progressive loading were applied to test the strength of different constructs. Stress and displacement data were predicted. Stress shielding and deformation change were used as criteria for biomechanical comparison. Stress shielding was least in the model, having 60 mm length and 13 mm width; the model having a divergent angled distal screw of 130° showed the least stress shielding in static loading, while one with a 90° screw set up showed the least stress shielding in progressive loading. The average stress and displacement of the osteoporotic femur implanted with DODO were 19 and 81% higher, respectively. A model with a 29% reduced volume of the base predicted better biomechanical characteristics.

Significance of nonlinearity consideration in finite element analysis for preclinical strength assessment of extramedullary femur plates

Abstract Accurate finite element (FE)-based evaluations demand realistic modeling of the bone-implant construct. For this, considering nonlinearity in the FE model sometimes becomes essential. Human femora exhibit nonlinear mechanical behavior, which could be attributed to material, large deformation, and boundary conditions. The nonlinearity due to material is specified as material nonlinearity, while due to large deformation or boundary conditions is specified as geometric nonlinearity. The objective of the present study is to comprehend the importance of incorporating nonlinearity into FE analyses of extramedullary plates used to treat proximal femoral fractures. Two extramedullary implants – one proximal femoral locking plate (PFLP) and another variable angle-dynamic hip screw (VA-DHS) - were fixated with a femur analog having a simulated intertrochanteric fracture. The constructs were analyzed dynamically for normal walking gait activity and sideways fall scenarios. The analyses were carried out by considering both geometric as well as total (geometric & material) nonlinearity, and the same were compared with the results from linear models. The results obtained from FE analyses of intact and implanted constructs were validated from in-vitro experimental data and FE analysis data reported in previous studies. The investigation revealed that a comprehensive nonlinear analysis, considering both geometric and material factors, can minimize the error percentage to less than 5% while comparing with the data obtained from the experimental approach. The PFLP implant was found to impart higher stiffness (~30%) as compared to the VA-DHS implant under both load cases of walking and sideways fall.

A preclinical model of post-surgery secondary bone healing for subtrochanteric femoral fracture based on fuzzy interpretations

Mechanobiology plays an essential role in secondary bone fracture healing. While the introduction of newer type of plates, e.g. locking plate (LP), is becoming increasingly popular for complex femoral fractures, the conventional technique involving dynamic compression plate (DCP) remains the standard choice. The difference between the two techniques lies primarily in their screw fixation mechanisms. The present study applied 3D dynamic fracture healing scheme modelled on a subtrochanteric femur fracture, regulated by both finite element (FE) analysis and Fuzzy logic control in order to understand the spatio-temporal healing phenomena for both LP and DCP. The study further examined the influence of the two screw fixation mechanisms in determining the comparative progression of fracture healing. The problem was solved iteratively in several healing steps running in loop and accordingly, the local tissue concentrations and material properties were updated. The predicted results accorded well with various previous experimental observations. The study found an initial delay in healing associated with DCP. However, as the healing progressed, there was no significant difference in overall callus modulus. The presented preclinical model may further help predict bone healing for different implantation techniques, and thus can serve as a non-invasive tool for evaluating relative merits of extramedullary plating techniques.
