The Airway scope® (AWS), the KingVision® (KV) and the McGRATH® (MG) are new indirect video laryngoscopes designed to facilitate effective and safe tracheal intubation under various conditions. However, there are few comparative studies as for performance in tracheal intubation attempted by inexperienced personnel. The purpose of this study was to evaluate success rates, time to intubation with use of these devices by inexperienced personnel in a simulated manikin difficult airway.
We report an adult patient with refractory chronic inflammatory demyelinating polyneuropathy (CIDP) who resisted conventional immunological therapies but could be successfully treated with interferon (IFN)-alpha 2a. A 56-year-old man was admitted to our hospital with progressive distal dominant muscle weakness and sensory disturbance of all extremities. He was diagnosed as CIDP based on laboratory examinations which revealed conduction block on peripheral nerve conduction studies and demyelinating findings on sural nerve biopsy. He was given several immunological treatments, including immunoadsorption therapy (IAT), oral corticosteroid, immunosuppressive agents such as cyclophosphamide and mizoribine, but without satisfactory efficacy although transient improvement of muscle weakness was observed for two weeks after IAT. A session of IAT every four weeks was therefore continued to maintain his symptoms for nine months. He was then treated with IFN-alpha 2a intramuscular injections every other day. Four weeks after starting of IFN-alpha 2a therapy, the worsening of his symptoms stopped and his muscle strength improved gradually to normal level without IAT, suggesting that IFN-alpha 2a may be effective in some patients with refractory CIDP. Further studies are needed to confirm the efficacy of IFN-alpha 2a for CIDP.
Bone metabolic markers were investigated in the male hemodialysis patients within and over 60 years old and the female hemodialysis patients within and over 50 years old.Our results indicated that bone metabolic markers were more increased in the lower values of serum estradiol in the male patients, but inversely were more enhanced in higher values of serum estradiol in the female patients.
An entry from the Cambridge Structural Database, the world’s repository for small molecule crystal structures. The entry contains experimental data from a crystal diffraction study. The deposited dataset for this entry is freely available from the CCDC and typically includes 3D coordinates, cell parameters, space group, experimental conditions and quality measures.
Dural arteriovenous fistula (DAVF) is occasionally associated with venous/sinus stenosis. In some cases, venous congestion and venous hypertension due to venous/sinus stenosis can induce clinical symptoms. The authors report a case of transverse-sigmoid sinus DAVF treated with transvenous embolization and venous/sinus angioplasty and stenting.A 46-year-old man presented with psychiatric symptoms and was diagnosed with Cognard type IIa+b DAVF of the left transverse-sigmoid sinus. Retrograde venous drainage was directed toward the right side via confluence. Venous/sinus stenosis in the right internal jugular vein and transverse sinus with supratentorial venous congestion were observed. DAVF was transvenously embolized, and venous/ sinus angioplasties were performed. Balloon angioplasty was performed for stenosis at the transverse sinus, and stent angioplasty was performed for stenosis at the internal jugular vein because the stenosis was not sufficiently dilated with balloon angioplasty. DAVF was totally obliterated, and the venous congestion immediately improved. The patient's clinical symptoms gradually improved postoperatively.Venous/sinus angioplasty and stenting should be considered as a treatment for venous/sinus stenosis associated with DAVF.
