We developed and implemented in multicenter setting the standardized approach for flow cytometric minimal residual disease (MRD) monitoring in childhood B-cell precursor acute lymphoblastic leukemia (BCP-ALL). Participation of multicenter group reference laboratories in several ring trial studies demonstrated high concordance rate between participants. Successful integration of one additional laboratory in the multicenter group has shown good level of our approach reproducibility. These results will allow implementing MRD detection in stratification system of pediatric ALL treatment protocols of Russia-Belarus multicenter group.

Plasma Level of Tumor Necrosis Factor-alpha (TNF-α) Correlates with Leukocytosis and Biological Features of Leukemic Cells, but not Treatment Response of Children with Acute Lymphoblastic Leukemia

AbstractWe have analyzed the clinical significance of the plasma level of tumor necrosis-alpha (TNF-α) detection in childhood acute lymphoblastic leukemia (ALL) at diagnosis. 49 patients (pts) with B-lineage ALL (3–17 years of age) and 30 healthy children (age-related control) were enrolled in study. The mean value of the level of TNF-α was 43.4 ± 8.1 pg/ml in ALL pts and 30.7 ± 3 pg/ml in control. In ALL pts the level of TNF-α positively correlated with blast cell count in peripheral blood (R = + 0.432; P = 0.008); negatively correlated with percentages of S-phase leukemic cells (R = - 0.446; P = 0.042) and culture-induced apoptotic cells (R = - 0.41 1; P = 0.057). Pts with TNF-α level above the median value were characterized by higher WBC count (P = 0.025), lower percentage of S-phase leukemic cells (P = 0.02) and tendency to lower percentage of apoptotic cells (P = 0.07) vs pts with TNF-α level below the median value. No significant association of the level of TNF-α with treatment response at days 8 and 15 or 3-year overall survival was defined. We conclude that the elevated plasma level of TNF-α is a useful marker to assess disease activity/progression, but not prognosis of childhood B-lineage ALL.KeywordsTNF-αPlasma levelChildhood B-lineage ALL

Comparative measurement of spontaneous apoptosis in pediatric acute leukemia by different techniques

Abstract Background To distinguish between subgroups of patients with acute leukemia, the rate of spontaneous (culture‐induced) apoptosis of leukemic cells was evaluated using five methods. Methods Leukemic cells (cells) from the bone marrow of children with acute lymphoblastic leukemia (ALL, n = 112) and acute myeloid leukemia (AML, n = 30) were cultured for 20 h in vitro. The level of apoptosis was detected by fluorescent microscopy after staining with acridine orange (AO) or by flow cytometry after staining using PI, JC‐1, the APO‐BRDU kit, or the AnnexinV‐FITC kit. Results ALL cells were significantly more sensitive to spontaneous apoptosis versus AML cells, as was detected by all methods. The least sensitive technique was apoptosis detection by sub‐G1‐peak/PI‐staining. No difference in the rate of apoptosis in cells was determined between T‐ and B‐lineage ALL patients. In patients with B‐lineage ALL, strong positive correlation existed between the level of cells with loss of mitochondrial membrane potential (JC‐1), chromatin condensation (AO), and externalization of phosphatidylserine (AnnexinV+PI+). The proportion of AnnexinV+PI– cells had no correlative link with any other apoptotic cell subpopulation. Conclusions We found different sensitivities of ALL and AML cells to undergoing spontaneous apoptosis in vitro. Detection of the early/intermediate, but not the late stage of apoptosis is of preferable for correct assignment of spontaneous apoptosis in pediatric acute leukemia. Cytometry Part B (Clin.
